Home Care Team Manager
We are seeking a dynamic and experienced professional to join our team as the Health Services Care Team Manager (HSCM). In this role, you will lead and support a team of Care Managers to ensure high-quality service delivery and compliance with all regulations governing the Executive Office of Aging and Independence (AGE) State Home Care Programs, grants, and contracts. This is a critical leadership position that combines program oversight, staff development, and cross-department collaboration to ensure older adults and individuals with disabilities receive the highest standard of care.
Schedule: This is a full-time (35 hour/week) position with a hybrid remote schedule (2 day/week in the office) and the option of a flexible 4-day work week (once training is complete.) No nights or weekends.
Compensation: Starting annual salary is $72,000. Being bilingual is not a requirement for this role, but we recognize it as an asset and offer an additional 6% bilingual designation for Spanish, Haitian Creole, Russian, Vietnamese, Cantonese or Mandarin. In this role you will be responsible for assessing, reassessing and monitoring consumers for need and eligibility for in-home services, recommending/approving service plans, providing information on available resources, and coordinating in-home/community services.

Job Activities
Key activities of the position are below. Additional duties may be assigned as required.
- Lead in supervising, mentoring, training, and hiring of HSCMs to ensure the effective and efficient management of casework
- Conduct a record review for compliance with applicable regulations
- Actively participate in interdisciplinary team meetings
- Participate in problem-solving of management issues and discussion of care management and policy-related issues.
Other Activities:
- Monitor staff retention, turnover, and satisfaction
- Cross-collaboration with all departments for best practices
- Coordinate and assist in the development and implementation of new initiatives and programs
- Assure compliance and monitor EOEA, contracts and Agency policies and procedures

- KPIs of direct reports
- EOEA quality metrics
Qualifications:
- Master's Degree preferred or Bachelor's Degree in human services, nursing, social work or related fields.
- Experience in working in/with Elder Services, Home Care program, Senior Care Options and/or disabilities preferred or a minimum of 2 years of experience in related field
- 2-5 years of management/supervisory experience in a human service/community-based setting
- Strong analytical, managerial, communication, and leadership skills
- Project management skills
- The ability to manage multiple high priority objectives/projects
- In-depth understanding of all available State/Community Programs
- Excellent interpersonal skills with demonstrated ability to communicate across all levels, including staff, board members and external partners
- Highly resourceful team player, with the ability to work effectively independently or as a team
- Demonstrates a willingness to take on new tasks with a general attitude that no task is too small, impossible, or cannot be improved
- Expertise in the following platforms: Microsoft 365 (Word, Outlook, Teams, Excel, and PowerPoint), Adobe Pro, Zoom and other web-based applications
- Experience working in a 501c3 organization and managing in a union environment strongly preferred
- Private transportation
Physical Requirements of the Job:
- Ability to lift, push, pull and carry objects weighing up to 25 pounds
- Ability to climb up and down stairs unassisted
- Ability to travel into the community
- Work requires regular standing, stooping, and bending
