Title: Mobile Case Manager, Shelter 

 

Sign on Bonus: Sign on Bonus of $600 (Half paid out at 3 months and the other half at 12 months). 

Salary Range: $24.04 - $26.44/hour, $50,000.00 - $55,000.00/annually

Description (Reason Position Exists): 

 

Skilled in trauma-informed care, Mobile Case Managers help youth and young adults work to secure community resources, and plan for the next step on their journey. 180 Degrees maintain emergency shelter, transitional living, and supportive housing for youth experiencing homelessness. All of 180 Degrees youth shelters (Brittany's Place, Hope House, Rochester Youth Shelter, and St. Cloud Youth Shelter) have a mobile case manager who works directly with youth in the community and youth exiting a shelter. The work includes direct care and outreach work to build connections with other youth-serving agencies. The shelter Mobile Case Manager will provide short-term crisis response, hold an active caseload, and provide outreach services to the community surrounding the youth shelter.  

 

This position does not have direct reports. This position is 40 hours per week; FTE 1.0.

 

Responsibilities: Duties and Responsibilities include the following. Other duties may be assigned.

 

  1. Support the emergency Youth Shelter and/or Transitional Living Program (TLP) in building effective community partnerships and outreach related to the youth experiencing homelessness in the community.   
    • Increase public awareness about shelter services by engaging stakeholders through presentations, meetings, work groups, social media activity, literature drops, and more.  
  • Develop relationships with schools, librarians, drop-in centers and other community partners who have knowledge or programming with youth experiencing homelessness outside of shelter.  
  • Maintain an updated list of youth-service providers and distribute to appropriate parties in shelter and in the community.  
  • Strengthened collaboration to yield new basic center funded referrals. 
  • Provide face-to-face outreach directly to youth in places where they congregate.  
  • Meet individual funding service requirements and documentation for federal/state licensing and grant requirements. 
  • Directly serve youth by managing an ongoing caseload of up to 15 youth.    
  • Coordinate with appropriate staff and service providers for successful entry into and/or transition out of shelter. 
    • Offer aftercare case management for 90 days to 6 months, providing goal/safety planning, referrals, navigation, advocacy, and financial literacy. 
  • Develop comprehensive case and transition plans tailored to youth and family needs. 
  • Facilitate flexible case management through various formats, including face-to-face, text, phone, and virtual interactions. 
  • Identify and refer youth to 180 Degrees services and other community resources, including a smooth transition to aftercare following discharge.  
  • Document client interactions accurately and promptly, adhering to licensing and grant requirements. 
  • Provide crisis intervention tools, such as food access, obtaining transportation, safety plans, housing advocacy, and financial empowerment for youth not in shelter. 
  • Actively contribute to the Youth shelter.
  • Demonstrate effective teamwork and collaboration with management and colleagues. 
    • Collaborate with supportive services for youth choosing to not be sheltered who could benefit from an internal referral.  
  • Ensure all services align with best practices for effective youth support. 
  • Collaborate with the shelter leadership to address support needs, recommend referrals, seek consultation, and establish new processes that better serve youth in aftercare or in need of services.  
  • Engage in the preparation and attendance of case consultations, ensuring inclusion of relevant community partners and shelter staff while respecting data privacy; and reporting on aftercare progress. 
  • Ongoing engagement to bring awareness to youth shelter service and increase of service referrals.  
  • Consult on identifying gaps in services related to referrals, outreach, and aftercare; provide suggestions for solutions and action planning.
  • Active participant as member of the shelter team, including potential on-call rotation and potential programming facilitation. 
  • Other duties as assigned. 

 

I will have performed my job well when I:

  1. Support the youth shelters through the development and maintenance of professional, effective relationships with community partners. 
    • Ensure people served receive high quality trauma responsive services, including required documentation of services.   
    • Utilized evidence-based practicesand cultural competence to participate in wrap-around services and case management consultation.  
    • Engage in community outreach
    • Increase public awareness of the resources provided by 180 Degrees; expand community networks
    • Be active in the community to provide face-to-face outreach and increase referrals.
  2. Maintain an active caseload.
  3. Coordinate with others to facilitate successful entry into and/or transition out of shelter. 
    • Provide aftercare case management for goal/safety planning, referrals, navigation, advocacy, and financial literacy. 
  4. Provide case management through various formats, including face-to-face, text, phone, and virtual interactions. 
  5. Document client interactions accurately and promptly, adhering to licensing and grant requirements. 
  6. Provide crisis intervention tools, such as food access, obtaining transportation, safety plans, housing advocacy, and financial empowerment for youth not in shelter. 
  7. Contribute to the youth shelter operations.
  8. Demonstrate effective teamwork and collaboration with shelter management and staff to address support needs, recommend referrals, seek consultation, and establish new processes that better serve youth in aftercare or in need of services.  
    • Collaborate with and provide internal referrals to the 180 Degrees Supportive Services team for youth choosing to not be sheltered.  
  9. Ensure all services align with best practices for effective youth support. 
  10. Participate in case consultations, ensuring inclusion of relevant community partners and shelter staff with appropriate documentation and accurate reporting for aftercare progress. 
  11. Increase the number of service referrals. 
  12. Identify gaps in services related to referrals, outreach, and aftercare; provide recommendations for improvements.
  13. Participate in shelter staff meetings, provide suggestions for process improvements, facilitate shelter programming activities as needed, and participate in on-call rotation as needed.

 

Qualifications: 

  • Minimum age: 21.
  • Bachelor’s degree in social work, social services, psychology, or a related field and two years of case management experience related to crisis, violence, trafficking, exploitation or homelessness preferred; relevant work experience may be substituted in lieu of education on a year-for-year basis.
  • Experience in the nonprofit sector preferred.   
  • Valid driver’s license and insurance; dependable transportation for traveling to community partners and events.
  • Ability to clear a Department of Human Services background screening.

  • Knowledge of violence, trafficking, homelessness, trauma responsive care, youth-work, parenting, teaching and case management best practices.  
  • Awareness of one’s own cultural identity, with an understanding of how bias and experience influences their work with diverse individuals. 
  • Thrives on direct engagement with families/caregivers, employing skills in safety planning, crisis intervention, conflict resolution and motivational interviewing. 
  • Ability to work independently and in a team.

  • Strong verbal and written communication skills.   
  • Computer skills including MS Word, PowerPoint, SharePoint, Outlook, and Excel.   
  • Excellent organization with attention to detail and time management.
