Job Summary
Under the general direction Director of Behavioral Health, the Integrated Behavioral Health Consultant is responsible for providing evidence-based behavioral healthcare assessments and clinical services to patients at the Gritman Medical Clinics for a variety of emotional and psychiatric problems using the Primary Care Behavioral Health framework.
Job Duties and Responsibilities
• Provide direct behavioral health services to clients of all ages in a primary care setting, including brief interventions, crisis management, and psychoeducation.
• Conduct screenings and assessments to identify behavioral health needs and develop appropriate care plans.
• Collaborate with medical providers and the care team to address the behavioral health needs of patients in a holistic manner.
• Offer case management services, connecting patients with community resources, support groups, and specialized care as needed.
• Upon licensure, this role may be utilized to provide individual therapy to patients referred to by a clinic's primary care provider.
• Maintain accurate and timely clinical documentation in the electronic health record (EHR).
• Participate in interdisciplinary team meetings to discuss patient cases and coordinate care.
Job Requirements
Required Education
A bachelor’s degree in social work or a related field is required.
Required Licenses
While not required at the time of hire, a strong preference will be given to candidates who are licensed at the master's level (e.g., LMSW, LCSW, LPC, LCPC) or are able to obtain licensure within the first year of employment.
Required Work experience
A minimum of 2 years’ work experience in social work, psychology or related field.
Must have experience working with Electronic Medical Records (EMR) software as a user knowledge of healthcare terms, conditions, roles, integrated behavioral health, and basic care principles.

• Proficient in Microsoft Office Suite (Outlook, Word, Excel, Teams, PowerPoint), email, and electronic health record (EHR) systems.
• Knowledge of medical terminology, crisis theory, and community resources.
• Capable of independent work with minimal supervision in both in-person and hybrid settings.
• Demonstrates innovation, flexibility, and willingness to learn, teach, and improve workflows—especially within Behavioral Health.
• Maintains accurate documentation and applies appropriate communication strategies to improve patient access and outcomes.
Preferred qualifications
• MS Social Work, Education, Psychology, Nursing, or another health-related field. Candidates could also be pursuing said graduate level degree.
• Master’s Level Licensure (e.g., LMSW, LCSW, LPC, LCPC)
• Previous experience with health IT systems and data reports
• Previous experience with mobilizing community resources, navigating patients through the healthcare continuum, and working with disparate populations.
• Knowledge of primary care ambulatory practice workflows, including integrated behavioral health.

Population served
Neonate, pediatric, adolescent, adult, and geriatric. All genders, race, ethnic, sociopolitical, demographic, marital and sexual preference groups, developmental and physical abilities, religions, and nationalities. Population may have impairments of any or all body systems and senses.
Physical Demands
Desk work involves prolonged, considerable sitting. Extensive computer work with repetitive hand/wrist motion activities. Must possess good manual and finger dexterity. Visual requirements include near and far acuity, depth perception, color differentiation, and focusing ability. Hearing, speech, and sense of touch essential.
Environmental Conditions
Typically work in an office environment. Weekday hours. Frequently required to function competently with multiple demands and tasks
