PAY RANGE: $26.92 to $32.90 hourly

Established in 2023, CommuniCare+OLE is the result of a union of two health centers with  deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

JOB SUMMARY/OVERVIEW: 

The Enrollment Specialist works under the supervision of the Enrollment Services Supervisor to inform, educate, and facilitate enrollment into third-party programs as appropriate to the eligibility status of patients and family members. This position acts as a resource for patients regarding various health care programs, assistance with the enrollment process, and appropriate referrals. The Enrollment Specialist works as part of a health care team including physicians, mid-level practitioners, medical assistants, and health educators to provide comprehensive care to a primarily low-income, non-English speaking population.

MINIMUM POSITION REQUIREMENTS: EDUCATION, EXPERIENCE, SKILLS/TRAINING 

  1. Education: 

·        College degree or high school diploma with 2 years related work experience required.

  1. Experience/Lived Experience: 

·        Experience in a social service agency, health care organization, or medical office preferred.

·        Knowledge of medical terminology and community resources.

·        Knowledge of California health care programs.

  1. Special Skills/Training: 

·        Strong verbal and written communication skills; bilingual (English/Spanish) required.

·        Strong computer and data entry skills; Microsoft Office, Outlook, Word, Excel, use of a web browser, and use of an Electronic Health Record system preferred.

·        Professional phone etiquette skills required.

·        Must certify and remain current in CPR.

 

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES 

1.

Interview patients for program eligibility for a variety of health care coverage programs including, but not limited to, Covered California, Medi-Cal, Healthy Families, Patient Assistance Program (PAP), Kaiser Permanente Child Health Program (KPCHP), and County Medical Services Program (CMSP).

2.

Assist patients with applications to various programs and in the collection of necessary documentation.

3.

Refer patients to out-stationed county Eligibility Workers for completion of the application process when appropriate.

4.

Educate patients, family members, and the community about various health coverage programs.

5.

Develop flyers, hand-outs, and other informational materials to facilitate the application process.

6.

Case manage patient accounts by tracking eligibility, status in the application process, and monitoring patients’ enrollment in programs.

7.

Provide data entry of patient information into tracking systems.

8.

Initiate or maintain Covered California Certified Enrollment Counselor status.

9.

Act as a resource to all clinic staff regarding third-party payor programs and public charge and immigration issues relating to health care benefits

10.

Assist in training staff on eligibility requirements, application process, and benefits of enrollment in third-party programs and health care coverage.

11.

Attend health fairs, meetings, and other community events to inform the community about the various health care coverage options.

12.

Work with all departments to identify patients that need to be screened for eligibility for different insurance plans and programs.

13.

Greet patients and drive a positive patient customer service experience.

14.

Create and update patient records in the electronic health record.

15.

Schedule appointments as needed, according to policies and guidelines.

16.

Work with Care Coordinators to identify uninsured patients who need specialty care and need to apply for charity care at hospitals or other healthcare facilities. Assist patients in filling out charity care applications and provide any follow-up as needed.

17.

Produce regular reports on activity and outcomes.

18.

Performs other duties as assigned.
