Basic Function: 

 

Responsible for supporting Community Day Services and Community Living Programs in all aspects of case management, including documentation, compliance with state and accreditation requirements, coordination of supports, linkages with agencies, and family involvement. Ensures consistency in service delivery and contributes to quality, person-centered outcomes for individuals served.

 

Reporting Relationship:

Reports To (Position Title): Director of Performance, Compliance and Quality Assurance

Supervises (Position Title): None

Schedule and On-Call Responsibilities:

Monday – Friday 8 AM – 4 PM. On-call responsibilities are limited. Required to attend overnight conferences and training.

 

Principal Duties/Responsibilities:

  • Does not carry a permanent caseload but may be temporarily assigned open or vacant cases as needed to ensure continuity of services.
  • Provides direct assistance in case management processes, applying a high level of knowledge and skill to ensure quality and consistency.
  • Ensures that individual case records are accurate, timely, and compliant with IDHS, licensing, and accreditation requirements.
  • Promotes person-centered planning processes when coordinating and developing individual program planning for persons supported.
  • Ensures program goals and objectives are achieved by monitoring regularly and obtaining feedback and input as appropriate.
  • Supports accurate and timely data collection, baseline tracking, and monthly progress reporting to inform program planning and compliance monitoring.
  • Observes individuals, reviews data and progress, and revises documentation based on individual need, preference, and performance.
  • Provides support on incident and accident injury reports related to individuals receiving services as necessary.
  • Ensures program activities are driven by individual service plans of persons supported.
  • Works collaboratively with team members, demonstrating a positive and supportive attitude toward all staff and individuals.
  • Maintains current knowledge in the field, including updates from state and federal accrediting agencies.
  • Identifies potential areas for improvement in case management documentation and processes and recommends adjustments to strengthen quality and compliance.

Qualifications:

Education: Bachelor’s degree in the field of human services (e.g., sociology, special education, rehabilitation counseling, psychology).

Experience: Minimum of two years of case management experience; experience in compliance or quality assurance preferred. At least one year of experience working directly with persons with intellectual and/or developmental disabilities.

Knowledge/Skills:

  • Commitment to supporting individuals with intellectual and developmental disabilities (I/DD) through person-centered and respectful approaches.
  • Able to promote a positive, collaborative, and inclusive approach among program personnel.
  • Strong interpersonal, verbal, and written communication skills.
  • Strong organizational skills and ability to manage multiple priorities.
  • Comfortable interacting with internal staff as well as external funding and accrediting representatives.
  • Proficiency in Microsoft Office applications (Excel, Word, PowerPoint) and working knowledge of electronic record management systems.
  • Ability to adapt to changing priorities and provide support across multiple program areas as needed.
  • Knowledge of Electronic Record Management Systems.

Mental/Physical Demands:

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Can deal with problems involving several concrete variables in standardized situations. Ability to read and comprehend instructions, correspondence, and memos. Ability to write correspondence. While performing the duties of this job, the employee is regularly required to stand and talk or hear. The employee is frequently required to walk, use hands to finger, handle, or feel, and reach with hands and arms. The employee must often lift and/or move up to 25 pounds.  Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

Environmental Conditions:

The noise level in the work environment is usually moderate. Work is performed primarily inside, but it requires occasional exposure to outside weather conditions. Employees are occasionally exposed to moving machinery parts, such as floor cleaning equipment and a shrink wrap tunnel, and to fumes from cleaning products.
