Summary
The Clinical Operations Support RN – HOPE is responsible for auditing HOPE (Hospice Outcomes and Patient Evaluation) records for accuracy, completeness, and regulatory compliance prior to submission to CMS. This role serves as a vital quality checkpoint, ensuring documentation reflects high standards of care and meets federal requirements. The RN works closely with clinical teams and the Compliance, Quality, and Informatics departments to support consistent and compliant HOPE documentation across the organization. By proactively identifying issues and supporting the field teams, this RN acts as a bridge between operations and compliance, helping Four Seasons sustain a culture of quality and integrity in hospice care.
Essential Duties & Responsibilities
- Conduct thorough audits of HOPE items (Admission, Update Visits, Discharge) to verify:
- All required data fields are completed accurately
- Responses align with CMS definitions
- Assists with HOPE record completion by ensuring:
- Z0400 attestation is properly completed and signed by the appropriate staff
- Z0500 is signed when the HOPE record is complete and ready to submit to CMS
- Provide timely feedback to clinical staff and leaders regarding identified documentation errors or omissions
- Identify documentation trends or gaps and escalate concerns to leadership
- Collaborate with Clinical Operations Support team to enhance processes and education
- Track and report audit outcomes/correspondence, using standardized tools and templates
- Assist in the development and delivery of education related to HOPE documentation expectations as needed
- Stay up to date on CMS HOPE guidance and related regulatory changes
- Participate in team meetings and organizational quality improvement activities
Knowledge, Skills, and Abilities
- Strong understanding of CMS hospice regulations and HOPE items
- Detail-oriented with strong documentation review and analytical skills
- Comfortable using EMRs and Microsoft Office Suite (Excel, Word, Outlook)
- Ability to support others while fostering a positive team culture
- Excellent attention to detail and ability to manage multiple priorities
- Strong written and verbal communication skills
- Ability to remain organized, responsive, and calm under pressure
- Proficient in Microsoft Office and clinical documentation platforms
- Able to interpret clinical records and audit findings to offer actionable guidance
Minimum Qualifications
Education:
- Active RN license in North Carolina required
- Bachelor’s degree in nursing preferred
Experience:
- Minimum 2 years’ clinical experience, preferably in hospice, palliative care, or compliance roles
- Prior audit or documentation review experience strongly preferred
Licensures & Certifications:
- Valid driver’s license and proof of automobile insurance
- Active RN license in North Carolina required

- Ability to lift, carry, push & pull up to 10 pounds, occasionally
