The Financial Management Case Worker is responsible for providing comprehensive case management and financial support services to vulnerable individuals experiencing or at risk of housing instability across the five boroughs of New York City. This role focuses on developing individualized budget and benefits management plans, supporting access to entitlements, and coordinating with GMHC program staff and external partners to promote long-term housing retention. Using a client-centered, trauma-informed approach, the Financial Management Case Worker also facilitates referrals to supportive services, advocates on behalf of clients with public agencies and landlords and implements housing stability interventions to promote overall well-being.

Essential Job Functions

The Financial Management Case Worker will be responsible for the following duties:


Client Documentation & Records Management:

  • Coordinate the retrieval of necessary documentation for client intake and enrollment.
  • Complete all required Social Security Administration (SSA) benefit and accounting forms for assigned clients.
  • Maintain accurate, confidential client records in compliance with agency and funder requirements.
  • Ensure timely mailing and/or delivery of essential documents to clients.


Client Communication & Advocacy:

  • Maintain regular contact and communication with clients’ Human Resources Administration (HRA) case managers/workers.
  • Submit follow-up requests to HRA for updated Rent Portion Responsibility (RPR) details and current lease documentation.
  • Serve as a liaison between clients, public agencies, and service providers to coordinate services and resolve housing or benefit issues.
  • Provide tenancy education and advocacy for clients facing eviction or housing-related challenges.


Intake, Assessment & Financial Planning:

  • Conduct comprehensive intake assessments to evaluate clients’ financial needs and eligibility for public assistance and benefit programs.
  • Develop individualized service plans in partnership with clients, focused on budgeting, savings, and benefits management.
  • Provide basic financial counseling to support clients in understanding their income, expenses, and benefit entitlements.

  

Case Management & Support Services:

  • Caseload Management: provide case management services as needed.
  • Monitor client progress and adjust plans as necessary to meet financial and housing stability goals.
  • Assist with applications and renewals for social security administration, subsidies and other financial assistance programs, including rental subsidies, utility assistance, and other relevant benefits.


Housing Stability & Utility Assistance:

  • Educate clients on the Terms and Conditions of Tenancy to promote housing retention.
  • Review and monitor utility bills on behalf of clients, intervening as necessary to prevent arrears or service interruptions.
  • Coordinate utility support and payment plans with appropriate agencies or programs.


Referral & Service Coordination:

  • Connect clients to GMHC programs and external providers based on identified needs.
  • Facilitate warm handoffs and ensure follow-through on referrals.


Compliance, Reporting & Documentation:

  • Ensure timely and accurate documentation compliance.
  • Maintain thorough records of financial transactions, case notes, and client interactions in compliance with program requirements.
  • Submit progress notes and other compliance reports.
  • Track housing goals via spreadsheets and generate outcome reports.
  • Enter data into internal tracking systems (e.g., eCOMPAS, TREAT, AWARDS) and complete time and effort reports as required.


Professional Development & Collaboration:

  • Stay current on housing regulations, public benefits, and financial support programs.
  • Attend and actively participate in trainings, case conferences, team meetings, and GMHC events.
  • Build and maintain interagency relationships to enhance service coordination.


Other Duties: Perform additional responsibilities as assigned by the supervisor.

Requirements

Special Skills and Knowledge


Required

  • Knowledge of SSA, HRA, housing subsidy programs, and low-income utility assistance programs (e.g., HEAP, One-Shot Deals).
  • Strong understanding of housing resources, entitlements, and case management strategies to support housing stability.
  • Experience working in a fast-paced, data-driven, performance-based environment.
  • Ability to work independently and think critically is required.
  • Exceptional active listening skills with the ability to communicate professionally and effectively both verbally and in writing.
  • Experience working with vulnerable populations, including individuals with HIV/AIDS, substance use disorders, and mental health challenges.
  • Ability to work effectively and efficiently within an interdisciplinary team.
  • Commitment to performing duties from a place of empathy and understanding.

Preferred

  • Background working with non-profit agencies or work on behalf of LGBTQ+ communities and/or people living with HIV is highly desirable.
  • Experience with other historically marginalized communities (in a professional or volunteer capacity) is also desirable.
  • Bilingual (English/Spanish) skills are a strong plus.


Education and Certification

  • High School Diploma, GED, or HSE required, with a minimum of two (2) years of experience working with individuals experiencing homelessness, housing instability, or other related social service programs.
  • A Bachelor’s degree in Social Work, Human Services, Public Administration, or a related field is preferred.


Technical Skills 

The ideal candidate should have the following technical competencies to effectively manage case documentation, reporting, and client services:


Intermediate proficiency in Microsoft Excel, including:

  • Data tracking and analysis for case management and program reporting.
  • Creating and managing spreadsheets to monitor client progress and deliverables.
  • Using formulas, pivot tables, and data validation to ensure accuracy in documentation.


Basic Proficiency in Microsoft Office Suite, including:

  • Outlook (email communication, scheduling, and coordination with service providers).
  • Word (creating case notes, service plans, and official correspondence).
  • PowerPoint (preparing presentations for internal and external stakeholders).


Experience with electronic case management and data tracking systems, such as:

  • eCOMPAS, AWARDS, or similar platforms for documentation and compliance reporting.
  • Data entry and management to ensure contract deliverables are accurately recorded.


Familiarity with government benefit systems and entitlement programs, including:

  • Social Security, Medicaid, SNAP, and Housing Assistance Programs.
  • Ability to navigate online portals and submit required client documentation.


Strong documentation and compliance skills, ensuring:

  • Accurate case notes and service plans in adherence to regulatory requirements.
  • Confidentiality and compliance with HIPAA and other legal protections.


Work Environment & Schedule

This is a hybrid position requiring a mix of:

  • In-office duties (team meetings, case conferencing, and reporting).
  • Remote work as determined by the supervisor based on program needs.
  • Participation in the annual AIDS Walk New York event is mandatory for all GMHC staff.


Physical Demands

  • While carrying out the responsibilities of this role:
  • Employee will be routinely required to sit, engage in conversation, actively listen, and take notes.
  • Additionally, the employee must regularly stand, walk, and navigate stairs.
