POSITION SUMMARY

The Community Health & Resource Navigator serves as a vital link between the healthcare system and the community, helping individuals and families access health services, understand care options, and navigate complex systems. This role combines the outreach and education strengths of a Community Health Worker with the coordination and system-navigation expertise of a Health Resources Navigator. The Navigator works closely with clinical teams, social service providers, and community stakeholders to promote health equity and improve outcomes for underserved populations.


ESSENTIAL FUNCTIONS

  • Build relationships with individuals, families, and community organizations to promote health education and access to care.
  • Conduct outreach activities including home visits, community events, and health fairs.
  • Identify and address barriers to care such as transportation, housing, food insecurity, and language access.
  • Provide culturally appropriate health education on topics such as chronic disease management, preventive care, and behavioral health.
  • Assist patients in understanding and following their care plans.
  • Support enrollment in Medicaid, SNAP, and other public assistance programs.
  • Collaborate with clinical teams to support patient follow-up and continuity of care. 
  • Help patients schedule appointments, obtain referrals, and access community resources.
  • Document interactions and maintain accurate records in the electronic health record (EHR).
  • Advocate for patient needs within the healthcare system.
  • Participate in training and professional development to enhance skills and knowledge.
  • Contribute to program evaluation and quality improvement initiatives.
  • Performs related or similar duties as required or assigned.

Requirements

SKILLS & ABILITIES

Education:

  • High school diploma or GED required, associate degree preferred
  • Certification as a community health worker required
  • Minimum of 2 years of experience in a healthcare setting preferred.


Experience:

  • Ability to coordinate, prioritize, and manage a variety of diverse and complex tasks and systems 
  • Strong analytical and critical thinking skills; ability to assess challenges and develop creative, effective solutions 
  • Exceptional organization skills and high level of attention to detail 
  • Ability to work independently and self-motivated 
  • Excellent verbal and written communication skills, including the ability to communicate effectively with a wide range of audiences 


Computer Skills:

  • Proficiency with Microsoft Office applications (Word, Excel, Outlook, PowerPoint).
  • EHR experience preferred
