Overview:
Responsible for supporting the Maternity Support Services (MSS) program by conducting outreach,
providing patient support, performing waived testing (e.g., pregnancy tests), and completing
administrative duties to ensure program compliance and promote optimal OB patient outcomes.
Responsibilities:
Description of Primary Responsibilities
1. Works as part of the Maternity Support Services (MSS) team to support OB patients,
improve access to care, and promote optimal birth outcomes.
a. Conducts MSS intakes, including collecting patient information and verifying
insurance coverage.
b. Performs waived testing, such as pregnancy tests, following program protocols.
c. Provides patient support and education to help patients follow their care plans in
collaboration with medical providers and the MSS Team.
d. Completes and maintains documentation, including scanning MSS documents to
ensure compliance with program requirements and guidelines.
e. Travels to remote clinic sites as needed to perform program-related activities.
Patient Support and Professional Responsibilities
a. Connects patients with community services and resources to meet their needs and
support positive outcomes.
b. Guides patients on accessing community services to promote self-sufficiency.
c. Provides referrals, verbally or in writing, to service providers and MSS team members.
d. Offers encouragement and support to patients and their families.
e. Builds and maintains relationships with community agencies relevant to program
functions and referral needs.
f. Prepares and submits weekly encounter reports to the supervisor by Monday at 12:00
p.m. to document productivity.
g. Performs other duties as assigned.
h. Promotes a positive work culture and upholds clinic policies and procedures,
including the WE CARE Standards of Behavior.
i. Maintains confidentiality at all times regarding CBHA activities, reports, financials,
patient health information, and other proprietary information specific to CBHA.

1. Requires strong organizational skills to manage multiple responsibilities, including MSS
intakes, waived testing, administrative duties, and patient support activities, while working
both independently and as part of a team.

Professional and Technical Knowledge:
1. Requires a High School diploma/GED or equivalent education and/or relevant work
experience. An Associate’s degree is not required but is considered beneficial.
2. Requires the ability to gain knowledge and training in patient support, community resources,
MSS program procedures, and patient education. A valid Community Health Worker (CHW)
certificate and Medical Assistant – Registered (MA-R) license will be acquired as part of onthe-job training.
Licenses and Certification:
Possesses a valid driver’s license and meets company insurance liability standards, as well as the
ability to travel to perform other work-related activities.
Technical Skills:
1. Ability to prepare basic correspondence and reports using Microsoft Word.
2. Ability to create basic spreadsheets and tables using Microsoft Excel.
3. Ability to develop basic presentations using Microsoft PowerPoint.
4. Ability to enter, update, and manage data in a centralized electronic health record system and
Microsoft Outlook.
5. Proficiency in scheduling and calendar management to coordinate patient appointments and
team meetings.
6. Ability to generate and interpret basic data reports to support program operations and
productivity tracking.
7. Professional use of electronic communication tools for patient and team correspondence.
8. Ability to scan, organize, and maintain electronic patient and program records in compliance
with program requirements.
9. Familiarity with telehealth or video conferencing platforms, if applicable.
10. Basic troubleshooting skills for minor computer, software, or equipment issues.
