About the Department

COLA_Department_Logo_RGB_FC_HomelessServices.jpgThe Case Manager supports participants in establishing and fulfilling their individualized treatment plans.   Evening hours and occasional weekend hours are sometimes required.

The work is considered sedentary and involves walking or standing some of the time and involves exerting up to 15 pounds of force on a recurring basis or routine keyboard operations. The work requires the following physical abilities to perform the essential job functions: hearing, mental acuity, speaking, talking, and visual acuity.

Work environment involves exposure to no known environmental hazards; and is relatively safe, secure and stable.

Position Duties

The tasks listed below are those that represent the majority of the time spent working in this position.  Management may assign additional tasks related to the type of work of the position as necessary.

  • Case manager must attend and complete initial HMIS Training;
  • Accomplishes participant’s care by assessing treatment needs: developing, monitoring, and evaluating treatment plans and progress;
  • Admits new participants by reviewing records and applications; conducting orientations;
  • Determines participants goals and objectives by completing intake interviews; determining need for therapeutic medical services, follow-up on assessments and recommendations after referrals are made to community partners;
  • Document all interaction with participants in the Homeless Management Information System (HMIS);
  • Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation and escort;
  • Communicates participates progress by engaging in weekly supervisor meetings and evaluations; disseminating results and obstacles to achieving goals and objectives;
  • Prepares participants discharge by reviewing and amplifying discharge plans; coordinating discharge and post-discharge requirements;
  • Daily random walk-throughs of the site to maintain a safe, comfortable, and neat environment for the participants;
  • Performs other related duties as required to include the flexibility to work occasional extra hours on nights and/or weekends to meet with clients that are unavailable during the day due to work or other relevant appointments;
  • Check voicemails daily and respond appropriately minimum twice a day, Monday through Friday, excluding state holidays; and
  • Performs other related duties as assigned. 

Minimum Qualifications

MINIMUM REQUIREMENTS TO PERFORM WORK: 

  • Bachelor’s degree in social work, counseling or comparable human services degree providing direct case management services;
  • HMIS Database experience preferred;
  • Valid South Carolina Class “D” Driver’s License.

Other Qualifications

  • Knowledge of personal computer equipment with skill in the use of Microsoft Office  preferred, utilizing Outlook, word processing, spreadsheet and presentation software programs;
  • Possess good interpersonal skills with the ability to build rapport with attention to maintaining appropriate boundaries and ethics;
  • Ability to express ideas clearly, both verbally and in writing following HR policies and laws;
  • Ability to think independently, work as a team, and to make ethical decisions;
  • Ability to problem solve utilizing critical thinking skills; and
  • Ability to listen and communicate effectively.
