SUMMARY

The Oncology Triage RN is responsible for providing professional nursing care for clinic patients following established standards and practices.  Assists physicians and staff in coordinating patient care. This individual functions as primary liaison between patients, staff and physicians.  Reports to Charge Nurse and Clinic Manager.

ESSENTIAL DUTIES AND RESPONSIBILITES 

Includes but not limited to the following:

  • Handles all patient triage phone calls by the end of business day.  All calls should be documented as the calls are handled.
  • Coordinates the processing of oral chemotherapy orders with the office dispensary staff and ensuring patient is in compliance with orders.
  • Provides patient education for all chemotherapy treatment orders.
  • Coordinates VNA orders with physician.
  • Provides the chemotherapy infusion department support as needed.
  • Assist physician with processing hospital orders
  • Provide assistance to physicians with patients referred from Stanford 
  • Performs other duties as assigned.
  • Applies the nursing process to plan and manage patient care, including meeting teaching and continuum of care needs.
  • Accurately perform an initial patient assessment, including a complete history and physical examination, and accurately performs ongoing patient assessment.
  • Nursing practice is consistent with all requirements of the house wide and department specific competencies.
  • Effectively delegates to and monitors the activities of support staff, and intervenes as needed by 

providing education and redirecting their activities as appropriate.

  • Informs patient of his/her role in providing accurate information and asking questions regarding diagnosis and treatments.
  • Patient care practice conforms with the established guidelines and preventative elements for infection control observed by:
  1. Utilization of Standard Precautions for all patients.
  2. Barrier use to protect self from potential exposures to blood and body fluid.
  3. Differentiation between medical waste and biohazardous waste streams.
  4. Utilizes prescribed barriers, signage, and patient education for each of the four transmission-based isolation categories.
  5. Complies with the Hand hygiene guidelines.

EDUCATION and/or EXPERIENCE

  • Current Registered Nurse License in the State of California
  • ONS Certification
  • Current CPR certification.

This job has no supervisory responsibilities.

  • Knowledge required to individualize the patient’s plan of care based on the diagnosis and/or condition/needs.
  • Oncology experience preferred.
  • Possesses a working knowledge of the Chain of Command Policy, including when and how the policy applies.
  • Possesses age-specific knowledge and skills required to complete an admission patient/family learning needs assessment.
  • Possesses the knowledge and skills required to develop an education plan based on the patient/family learning needs assessment.
  • Possesses the knowledge and skills required to safely manage all aspects of medication use processes including receiving telephone orders, transcribing and verifying new orders, administering medications, monitoring the effects, and reporting errors or other adverse events.
