The Quality Assessment/Performance Improvement Coordinator, RN is responsible for the quality performance of the organization, including the planning, gathering, monitoring, analyzing and reporting of all data that represents the agency’s performance improvement and patient safety monitors and outcomes for regulatory and internal purposes. Also responsible for oversight over all infection control related activities within the organization.


RESPONSIBILITIES:

  • Assists in the development of all QAPI activities and education with appropriate data collection, aggregation, analysis, taking action and reporting of results according to agency’s QAPI plan
  • Reviews patient medical records for compliance with federal, state and local laws, accreditation standards and agency policies and guidelines
  • Chairs QAPI committee, prepares QAPI reports/minutes of meetings, forwards reports according to QAPI plan
  • Responsible for weekly and monthly chart compliance audits
  • Understands and promotes principles of continuous QAPI
  • Responsible for the orientation of all new direct care staff go their job description, tasks, and responsibilities
  • Responsible for the orientation of all new staff to the QAPI program
  • Monitors needs and provides ongoing training and education to the staff
  • Monitors quality reports and provides support and education to assure ongoing quality standards and family satisfaction
  • Assists in the preparation and implementation of policies and procedures which meet Medicare and Medicaid accreditation standards, and state and local laws
  • Participates in studies and other administrative functions assigned
  • Serves as a resource to employees
  • Attends pertinent continuing education programs other than routine in-services and shares information with staff
  • Collects and reports HIS, HOPE and HCAHPS data for the agency
  • Participates in the leadership team
  • Participates in the Administrator on Call rotation
  • Assists HR to assure that staff is adequately credentialed to support the facility contracts
  • May perform patient care as a result of staffing levels
  • Responsible for coordinating the infection control program which includes surveillance; analysis of data; developing reports, policies and procedures; participates in infection prevent and control education of all employees during orientation and annually thereafter
  • Develops and implements continuously improved patient care procedures and control mechanisms relating to quality, compliance and infectious diseases
  • Develops and executes annual competency program for all clinical employees
  • Performs other duties as assigned by Executive Director

Requirements

  • Graduate of an approved school of professional nursing and currently licensed in the state of Georgia
  • Minimum of 5 years nursing experience, registered nurse experience preferred
  • Minimum of 5 years hospice experience
  • Minimum of 1-year quality assessment/performance improvement (QAPI) in a hospice setting
  • Ability to exercise initiative and independent judgment
  • Ability to enlist cooperation of many people to perform/achieve a common goal
  • Problem-solving and conflict resolution skills to foster effective work relationships with team members
  • Proficiency in Microsoft Office
  • Current CPR certification
  • Current Driver's License and vehicle insurance, and access to a dependable vehicle
