What You'll Do:
In the Clinical Case Manager role, you’ll provide high-touch, community-based recuperative care (medical respite) services to individuals with serious and complex behavioral health and social service needs who are experiencing homelessness. This role supports clients in achieving or maintaining individual stability and prevent hospital admission or readmission and system disengagement by providing persistent outreach, advocacy, and coordination of care across multiple service domains. The position focuses on improving functioning, enhancing quality of life, and promoting long-term stability to prevent returning to homelessness.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

- Provide comprehensive intensive case management services to individuals who have serious behavioral health conditions, including substance use disorder and experiencing homelessness utilizing trauma informed and harm reduction principles and practices
- Conduct program intakes, comprehensive bio-psychosocial needs assessment and service evaluations
- Develop, implement and regularly update collaborative care management and housing support plans which support individuals’ needs in the areas of physical health, mental health, substance use disorders, community-based services support, social supports, and social determinants of health
- Engage in frequent client contact through scheduled or unscheduled visits, in program outreach, and telephonic support—often multiple times per week.
- Advocate for clients to reduce barriers to care and ensure access to benefits and community resources including accessing additional benefits and related documentation (i.e. Social Security Insurance (SSI), CalFresh, cash aid, and obtaining required documentation (ID, birth certificate, social security card, financial records, etc.)
- Collaborate with community partners, legal guardians, law enforcement, and healthcare providers as appropriate.
- Facilitate transitions of care, including hospital discharge planning, linkage to housing, and support during system navigation.
- Coordinate and integrate services across medical, mental health, substance use, housing, and social service systems.
- Provide crisis intervention, de-escalation, and safety planning as needed.
- Assist clients with life skills education, appointment scheduling, and transportation coordination.
- Monitor and document client progress, including changes in clinical status and service needs.
- Conduct discharge planning to appropriate community housing including application assistance, linkage to resources and referrals to facilitate independence and transition to stable housing.
- Collaborate and communicate effectively with an interdisciplinary and external care team – often multiple times per day
- Coordinate care with managed care plans (MCP), community providers, other referral sources and caregiver/authorized support person(s)
- Participate in case reviews, multidisciplinary team meetings, and quality improvement initiatives.
- Maintain accurate, timely, and confidential documentation in compliance with agency and regulatory standards.
- Maintain a flexible schedule according to client needs and in communication with program leadership
- Participate in all staff meetings and trainings as requested
- Perform additional duties as assigned
Who You Are:
You're passionate about making a lasting impact, whether supporting individuals directly or enhancing our organizational effectiveness. To thrive in this role, you'll:
- Master’s Degree in Social Work (MSW), Master’s Degree with an emphasis in Marriage and Family Therapy (MFT) or related field, required
- License-eligible, and currently registered and in good standing with the State of California Board of Behavioral Science as an Associate Social Worker (ASW),
- Associate Marriage and Family Therapist (AMFT) or related field, preferred
- Two (2) or more years of relevant work experience, required
- Knowledge of substance use, mental health, assessments, intervention and coordinated care strategies in a community non-profit setting, preferred
- Bilingual (Spanish/English), preferred

- Strong clinical judgment and ability to manage high-acuity cases.

- Working knowledge and skills in mental health, co-occurring disorders, substance abuse treatment models, and crisis intervention

- Must have excellent written and verbal communication skills
- Must be highly organized and demonstrate attention to detail
- Demonstrated experience managing multiple deadlines and working effectively under pressure, using independent judgment to produce quality work under time constraints
- Demonstrated experience working with a collaborative team
- Working knowledge of MS Office (Word, Excel, and Outlook)
- Ability to communicate effectively among a diverse workforce population and maintain sound working relations with all levels within the organization
- Work with spirit of teamwork, cooperation and a sense of urgency
- Maintain a high degree of confidentiality over all matters in the course of business operations including client and employee information
- Ability to multi-task in an efficient, thorough, and prioritized manner
- Ability to work quickly, accurately and independently

Location: Onsite/Escondido
Hourly Rate: $33.00-$38.00
Hours: Monday-Friday: 10:00am-6:30pm
