UCP Heartland is looking for a Family Navigator who will serve as a compassionate and informed advocate for individuals with developmental disabilities and their families, supporting access to resources, services, and information that promote independence and quality of life. This position acts as both a systems navigator and intake coordinator, ensuring families receive comprehensive and person-centered support when entering UCP Heartland services. The Navigator provides intake services, conducts needs assessments, connects individuals to internal and external programs, and facilitates successful service transitions. This role is essential to helping clients and families navigate complex service delivery systems while promoting equity, inclusion, and empowerment
WHAT IS THE JOB COMPOSED OF?
Client Navigation & Advocacy
- Serve as a central point of contact for families, caregivers, and individuals seeking services.
- Support clients in identifying needs and accessing appropriate supports including housing, medical, financial, educational, and behavioral health services.
- Provide information, referrals, and education on available resources and how to navigate public and private systems.
- Facilitate monthly self-advocacy meetings and contribute to the development of leadership among individuals served.
- Conduct follow-up calls with clients in UCPH residential and program services to assess satisfaction and identify additional needs.
Intake Coordination
- Manage incoming inquiries across programs (ABA, Daycare, Navigation, Employment, Residential, Adult Day Services, ITT, Youth Camps, and ISLA) through phone, email, and website submissions.
- Conduct preliminary screenings and gather documentation to determine eligibility and match individuals with appropriate services.
- Coordinate with internal program teams for service assignment, follow-ups, and warm hand-offs.
- Track inquiry-to-enrollment data and provide insight into referral patterns and capacity.
Collaboration & Outreach
- Collaborate with UCPH departments (e.g., ABA, Business Office, Residential, Development) to ensure seamless intake and navigation support.
- Represent UCPH at community events, transition fairs, and public information sessions.
- Contribute to Disability Rights Legislative Day, Children Disability Rights Day, etc. planning and other advocacy efforts.
- Documentation & Reporting
- Maintain thorough, timely documentation of client interactions, service plans, and resource connections.
- Upload necessary client records and assessments to designated systems.
- Participate in quality improvement initiatives related to intake and navigation services.

MINIMUM EMPLOYMENT STANDARDS:
- High school diploma or GED required, bachelor’s degree in human services, psychology, special education, or related field preferred.
- One or more years of experience working with individuals with developmental and/or physical disabilities.
- Strong communication skills and ability to connect with a diverse population of families and professionals.
- Familiarity with local and state service systems, including Medicaid, DMH, and special education services.
- Proficiency in Microsoft Office Suite and willingness to learn agency-specific databases (SetWorks, Central Reach, etc.).
- Reliable Transportation with Valid Driver's License
PREFERRED EMPLOYMENT STANDARDS:
- Bachelor’s degree in special education, psychology, social work or other related human services field from an accredited college or university or a combination of education and experience.
- Knowledge of or experience with state and local service delivery systems and resources for persons with disabilities and qualifications for acceptance.
This is a Full-Time, Salaried, EXEMPT Position, ON SITE
$42,000 to $46,000 Annually
