In this role, you’ll provide support to ensure seamless care coordination, efficient operations, and an exceptional patient experience.

Location: Post Acute department, Remote schedule but must be located in Buffalo or surrounding areas
Position Type: Full-time, Day Shift (Monday–Friday, 8:00 a.m.–4:30 p.m.)

What You’ll Be Doing:

The Patient Care Navigator serves as a key point of contact between patients, providers, and facilities—helping coordinate care, manage documentation, and support daily clinical and administrative operations.

  • Answer phones and route communications appropriately, schedule appointments, and document interactions in the EMR system (Medent/PCC).

  • Complete chart preparation for specialty appointments and coordinate schedules for providers across Assisted Living and Skilled Nursing Facilities.

  • Complete prior authorizations for medications and imaging services to ensure timely care.

  • Review and process faxes, medical record requests, and triage items throughout the day.

  • Send Transition of Care (TOC) discharge summaries to appropriate departments, including GLIN nursing staff.

  • Maintain a safe, secure, and compliant work environment by following established policies and procedures.

  • Deliver exceptional customer service to provide a positive and supportive patient experience.

  • Manage incoming document bins, assist with the completion of forms, and support the team as needed.

  • Work independently and collaboratively to ensure efficient operations and continuity of care.

Additional Responsibilities:

  • Provide coverage and assistance when teammates are out.

  • Participate in staff meetings and one-on-one sessions.

  • Perform other duties as assigned by management.

The Education and Experience You Need:

  • Education: High school diploma required; certification or degree in a healthcare-related field preferred.

  • Experience: Minimum of 3 years of experience in a healthcare setting preferred.

  • Knowledge: Familiarity with Medent EMR and healthcare documentation preferred.

  • Skills: Strong written and verbal communication skills, attention to detail, and proficiency with computer systems.
