In this role, you’ll provide support to ensure seamless care coordination, efficient operations, and an exceptional patient experience.
Location: Post Acute department, Remote schedule but must be located in Buffalo or surrounding areas
Position Type: Full-time, Day Shift (Monday–Friday, 8:00 a.m.–4:30 p.m.)

What You’ll Be Doing:
The Patient Care Navigator serves as a key point of contact between patients, providers, and facilities—helping coordinate care, manage documentation, and support daily clinical and administrative operations.
Answer phones and route communications appropriately, schedule appointments, and document interactions in the EMR system (Medent/PCC).
Complete chart preparation for specialty appointments and coordinate schedules for providers across Assisted Living and Skilled Nursing Facilities.
Complete prior authorizations for medications and imaging services to ensure timely care.
Review and process faxes, medical record requests, and triage items throughout the day.
Send Transition of Care (TOC) discharge summaries to appropriate departments, including GLIN nursing staff.
Maintain a safe, secure, and compliant work environment by following established policies and procedures.
Deliver exceptional customer service to provide a positive and supportive patient experience.
Manage incoming document bins, assist with the completion of forms, and support the team as needed.
Work independently and collaboratively to ensure efficient operations and continuity of care.
Additional Responsibilities:
Provide coverage and assistance when teammates are out.
Participate in staff meetings and one-on-one sessions.
Perform other duties as assigned by management.
The Education and Experience You Need:
Education: High school diploma required; certification or degree in a healthcare-related field preferred.
Experience: Minimum of 3 years of experience in a healthcare setting preferred.
Knowledge: Familiarity with Medent EMR and healthcare documentation preferred.
Skills: Strong written and verbal communication skills, attention to detail, and proficiency with computer systems.
