Position Title: Senior Service Coordinator
Department: Case Management
Reports to: Service Coordinator Supervisor
FLSA: non-exempt

Starting Rate: $24.85 - $25.81

General Description:
The Senior Service Coordinator provides support to a team of Service Coordinators through mentorship and hands on support. This position will act as a mentor for new staff joining the team and ensure training is received to complete critical components of the SC position. This training will include hands on, classroom, observation and accountability as well as organization of the new staff’s responsibilities. Sr. Service Coordinators need to aspire and develop skills for leadership roles within the organization and will provide coverage for the team supervisor when needed. Sr. Service Coordinators will continue to have a caseload of people they support, those enrolling into services and when coverage is needed. The Service Coordinator job description will be followed for those needs.
Essential Duties/Responsibilities:
- See responsibilities of a Service Coordinator as that job description applies to the management of a caseload. Caseloads will typically be smaller than that of a full time Service Coordinator.
- Train new SCs coming into the department and ensure they receive critical information and are able to apply it prior to doing their work independently. Review the work of staff new to the team and when needed for others. Assist in identifying gaps in learning or training needed to help improve the quality of work being performed by team members.
- Mentor staff so that they might assume greater responsibility and autonomy.
- Assist in identifying strengths and assists the staff person in capitalizing and sharing those strengths.
- Complete and document billable activities according to the Contract, CMS and HCPF rules, as appropriate.
- Articulate and encourages enthusiasm for a shared vision, mission, and/or change initiative along with guiding the performance of others, holding them accountable for their actions and sets good example for team members.
- Develop and maintain effective working relationships with others including other TRE employees, providers, families and community members. Represent TRE professionally. Build positive networks with others.
- Act as a resource is an area of expertise as needed by the team.
- Be effective with managing conflict and difficult situations. Handle emotions of self and others well, maintain self-confidence and encourage others to do so as well.
- Have good working knowledge of all systems used by TRE for business to include databases and video conferencing options.
- Assure compliance of team with TRE Policies and Procedures, protocols, deadlines, and applicable rules and regulations.
- Initiate change effectively and adapts to necessary changes in operations; and has a positive attitude towards operational changes.
- Promote a positive work environment and address conflict appropriately.
- Communicate clearly, listen effectively, keep others informed of necessary and factual information, and avoid gossip.
- Participate in internal and external professional development opportunities and projects based on personal professional goals/interests.
- Demonstrate knowledge of methods, techniques, and skills required in ongoing Service Coordination functions.
- Demonstrate coaching and mentoring by meeting with staff and by attending and observing interactions between them and others.
Other Duties/Responsibilities:
- Have an understanding of all governing and partnerships in the service delivery system.
- Meet with supervisor at least monthly and submit weekly and monthly paperwork/updates as requested.
- Ensure timely processing of paperwork and deadlines are met.
- Participate in committees and projects as needed and/or as interested.
- Other duties as assigned.
Job Qualifications:
Education and Experience Requirements:
Minimum: A Bachelor's level degree of education, five (5) years of experience in the field of developmental disabilities, or some combination of education and experience appropriate to the requirements of the position (experience must be specifically documented and verifiable on your application.) If an individual does not meet the educational requirement, considerations may be made if the individual has experience working with long-term services and supports (LTSS) population, in a private or public agency, or has a combination of LTSS experience and education, demonstrating a strong emphasis in a human behavioral science field. Experience in the Colorado DD system. Experience as a Service Coordinator at TRE, achieved the higher level of pay for SC responsibilities and has maintained Independent Level of Service Plan review. Excellent written and verbal communication skills. Strong advocacy skills.
Preferred: One (1) year experience at TRE. Graduation from an accredited college or university with a Bachelor’s degree in Sociology, Psychology, Education, Human Services, or other closely related field. At least two years of experience in a case management environment. Experience working with families and community resources; knowledge of family dynamics and resource development. Working knowledge of Medicaid Waiver programs and requirements; experience working with persons with disabilities in a community, employment, or recreational setting. Supervision or management experience helpful.
Material and Equipment Directly Used: Computers/tablets, Printers, Copy Machines, Personal Vehicle, Cell phones/telephone, Cloud based systems and software, etc.
Working Environment/Physical Activities: Office, community, private residences, commercial properties, and virtual environment settings. Moderate physical activity required, such as walking, sitting, standing, and lifting up to 20 pounds (ADA accommodations at the request of the employee). A working vehicle, valid CO driver’s license (unless active Military family), and auto insurance coverage with bodily injury and property damage liability coverage ($100,000/$300,000/$100,000 coverage recommended) are required.
