Position Title: Ongoing Coordinator Supervisor
Department: CMA – Adult Services
Reports to: Ongoing Program Services
FLSA: Exempt

Starting Rate: $63504.62-- $66560.50 annually (Rates reflect FY24-25 and are subject to change; contact [email protected] to confirm current rates.)

General Description:
The Ongoing Coordinator Supervisor (OCS) provides supervision and operational leadership to a team of Ongoing Service Coordinators delivering case management for individuals enrolled in Home and Community-Based Services (HCBS) waivers for both intellectual and developmental disabilities (IDD) and non-IDD populations (e.g., Elderly, Blind, and Disabled [EBD], Community Mental Health Supports [CMHS], Brain Injury [BI], Spinal Cord Injury [SCI], Supported Living Services [SLS], and Developmental Disabilities [DD] waivers).
The OCS ensures compliance with the Colorado Department of Health Care Policy and Financing (HCPF) Rules, Regulations, and the expectations of Case Management Redesign (CMRD) under 10 CCR 2505-10. The OCS oversees performance, provides technical assistance, ensures adherence to person-centered practices, and supports continuous improvement efforts to ensure timely, accurate, and high-quality case management.
Essential Duties and Responsibilities:
- Regulatory and Program Oversight:
Maintain comprehensive knowledge of Medicaid LTSS rules, waiver-specific regulations, HCPF memos (e.g., Policy and Operational Memos), and ensure assigned Ongoing Coordinators are compliant with all requirements related to:- Annual functional assessments (e.g., ULTC 100.2)
- Person-centered service plans
- Critical incident monitoring and follow-up
- Timely PAR submissions
- Transitions of care and disenrollment procedures
- Supervision and Support:
Provide direct supervision for a team of up to 15 Ongoing Coordinators, ensuring consistent coaching, accountability, and performance tracking. Use real-time feedback, supervision meetings, and development plans to support performance improvement and retention. - Quality Assurance and Documentation Review:
Routinely audit case files, service plans, notes, and system entries in state platforms such as CCM, BUS, and Bridge to ensure data integrity and compliance. Provide coaching and training when documentation or processes fall below HCPF standards. - Training and Professional Development:
Deliver onboarding and ongoing training to ensure Coordinators remain updated on changes to waiver guidance, system functionality, and evolving best practices in case management. Encourage and support staff in attending required HCPF trainings. - Case Management Redesign Compliance:
Lead internal process improvements in alignment with CMRD, including standardizing workflows for multiple waiver populations, cross-training teams on all waiver types, and ensuring timelines are met for initial contact, LOC assessments, and plan development. - Collaboration and Communication:
Serve as a liaison between TRE’s case management leadership and HCPF or external partners to resolve escalated concerns, submit requested documentation, and participate in corrective action plans as needed. - Team Engagement and Culture:
Foster a trauma-informed, inclusive, and strength-based team culture that reflects TRE’s values. Promote employee wellness, collaboration, and shared responsibility for meeting program and regulatory goals. - Conflict Resolution and Risk Management:
Support resolution of team conflicts or escalated participant concerns. Implement proactive strategies for identifying risks and applying early intervention to improve team functioning and service delivery. - Data and Metrics Tracking:
Monitor and report on performance metrics such as timeliness, contact frequency, documentation completeness, and audit outcomes. Use these insights to drive team discussions, performance plans, and improvement strategies. - Meetings and Leadership Participation:
Facilitate weekly team meetings and participate in department, inter-agency, or HCPF-led workgroups to represent Ongoing Coordination practices and ensure agency-wide alignment.
Other Duties and Responsibilities:
- Provide supervisory or case coverage during vacancies or staff absences.
- Participate in goal-setting aligned with TRE’s strategic priorities and HCPF's reform goals.
- Remain adaptable to last-minute changes in state/federal guidance affecting policy and operations.
- Support hiring and interview processes for new team members.
- Perform additional duties as assigned in support of team stability and participant well-being.
Job Qualifications:
Knowledge, Skills, and Abilities:
- In-depth knowledge of HCBS waiver programs, including eligibility criteria, service planning, critical incident management, and HCPF regulatory requirements.
- Proficiency in documentation and case management systems (BUS, CCM, MMIS, Bridge).
- Ability to train and coach staff in interpreting and applying complex rules and regulations.
- Familiarity with person-centered planning principles and conflict-free case management.
- Strong organization, time management, multitasking, and communication skills.
- Cultural awareness and understanding of social determinants of health.
- Competency with Microsoft 365, Outlook, Teams, Excel, and secure documentation systems.
Education and Experience Requirements:
- Minimum: Bachelor’s degree in a human behavioral science or related field. If an individual does not meet the educational requirement, they must have demonstrated experience working with the LTSS population and Medicaid-funded programs.
- Preferred:
- Two (2) years of case management experience in IDD and/or non-IDD waivers.
- One (1) year of supervisory or team lead experience in a healthcare, behavioral health, or social services setting.
- Experience using Colorado's state case management platforms and knowledge of the Case Management Redesign transition.
Material and Equipment Directly Used: Computers, Printers, Copy Machines, Surface Pro/tablets, Cell phones/telephone, Cloud based systems and software, etc.
Working Environment and Physical Activities: Office, community, private residences, commercial properties, and virtual environment settings. Settings include occasionally working with or in proximity to people who are or were incarcerated for a variety of crimes which may include violent crimes and sex offences. Extended time on the computer will regularly occur. Moderate physical activity required, such as walking, sitting, standing, and lifting up to 20 pounds. A working vehicle, valid CO driver’s license (unless active Military family), and minimum insurance levels ($100,000/$300,000/$100,000 liability coverage) are required.
