Pay Rate/Benefits: The pay range for this role is $76,500-$81,500 annually, commensurate with relevant experience and qualifications and in alignment with internal equity. Community Health Action of Staten Island (CHASI) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. POSITION SUMMARY:
The Associate Vice President (AVP) of HIV Care Coordination Services is a senior leader responsible for the strategic oversight, operational excellence, and programmatic integrity of CHASI’s HIV care coordination portfolio. This role ensures the successful implementation of grant-funded programs, alignment with funder expectations, and achievement of measurable health outcomes. The AVP leads a multidisciplinary team, including program directors, counselors, and patient navigators, fostering collaboration across roles to deliver integrated, client-centered care. The AVP also represents CHASI in key partnerships and public forums to advance health equity for people living with HIV.
CHASI’s HIV Care Coordination Department includes three integrated programs: the Care Coordination Revised Program, which ensures timely linkage to care, retention, treatment adherence, viral suppression, and reduced HIV-related mortality; the Ryan White Support Services Program, which promotes health and self-management through case management, psychosocial support, food assistance, and health education; and the Supportive Counseling Program, which delivers evidence-based trauma interventions and coping strategies to support overall wellness. Together, these programs are designed to improve medical outcomes, achieve viral suppression, and empower clients with self-management skills.
DUTIES & RESPONSIBILITIES:
- Provide strategic and operational leadership for CHASI’s HIV services, ensuring fidelity to program models and achievement of outcomes.
- Oversee program performance, quality assurance, and compliance with funder and regulatory requirements.
- Lead and support program staff through recruitment, supervision, professional development, and performance management.
- Deliver clinical supervision and promote trauma-informed, client-centered care practices.
- Serve as the primary liaison with funders, government agencies, and community partners; represent CHASI in coalitions, task forces, and public forums.
- Collaborate with the Vice President of Programs to develop and execute annual work plans aligned with organizational and funder goals.
- Ensure timely and accurate reporting of program data, deliverables, and outcomes.
- Drive continuous improvement through data analysis, contract management collaboration, and strategic planning.
- Champion CHASI’s mission, values, and commitment to health equity in all aspects of the role.
- Perform other duties as assigned.
QUALIFICATIONS:
- Master’s degree and licensure in social work, mental health counseling, public health, or a related field.
- Minimum 4 years of experience in HIV services or related health equity work, with at least 3 years in a supervisory or program management role.
- Proven ability to lead teams, manage complex programs, and drive measurable outcomes.
- Strong communication, analytical, and organizational skills.
- Experience with contract compliance, data reporting, and quality improvement.
- Proficiency in Microsoft Office and data systems; ability to learn new platforms quickly.
- Must reside within a two-hour radius to respond to emergencies in person, if needed.
