Position Status: Full Time

FLSA Status: Non-Exempt

Pay & Benefits: The pay range for this role is generally $41,600 - $45,000  annually, commensurate with relevant experience and qualifications and in  alignment with internal equity. Community Health Action of Staten Island (CHASI) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community


Position Summary


The Community Health Worker supports individuals and families in accessing essential services that promote stability, and wellness. Based at CHASI’s Pantry, this position serves as a referral specialist within CHASI’s Social Care Network under New York State’s 1115 Medicaid Waiver. The role focuses on connecting clients to a wide range of supports—including food assistance, utilities, housing, pest remediation, transportation, and other social care services. The Community Health Worker ensures referrals are completed efficiently, barriers are addressed, and clients receive the resources needed to improve their overall well-being.


Key Responsibilities


• Conduct intake and needs assessments for clients seeking assistance through CHASI’s Social Care Network.
• Coordinate and track referrals for social care services such as food, housing, utilities, pest remediation, transportation, and home   modifications.
• Provide short-term case management and follow-up to ensure clients receive referred services and that needs are met promptly.
• Maintain strong communication with referring staff, internal teams, and partner agencies to ensure continuity of care and  positive client outcomes.
• Maintain accurate, timely documentation in all required data systems.
• Prepare and submit reports as required by program leadership and funders.
• Participate in case discussions, supervision, and team meetings.
• Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values.
• Other duties as assigned.



Requirements

 

Qualifications  • High School Diploma or Equivalency required; college degree preferred.  • Experience in referral coordination, case management, or community health work preferred.  • Strong communication, engagement, and organizational skills.  • Ability to navigate health and social service systems and coordinate across multiple providers.  • Proficiency with data entry and electronic documentation systems.  • Knowledge of social care resources and community supports available in New York City, particularly Staten Island.  • Bilingual (English/Spanish) strongly preferred.  • Position is primarily office-based at CHASI’s Pantry location.
