PLEASE NOTE: THIS POSITION IS FOR EIGHT (8) HOURS PER MONTH FOR A REGISTERED NURSE

Title: REGISTERED NURSE (RN)

General Statement of Function:

The Registered Nurse (RN) provides comprehensive person-centered care, support, and consultation services for staff and participants. This position is responsible for training, monitoring, and coaching healthcare practices to ensure optimal functioning and maintenance of good health for all participants. This role includes program oversight, care plan development, staff training, and acting as a liaison with medical professionals and families.

Essential Functions:

Program Oversight

  • Provide program oversight at a minimum of 8 hours per month.
  • Provide care plan oversight in the absence of the Licensed Vocational Nurse (LVN).
  • Conduct quality assurance checks to ensure provided services meet standards and guidelines.
  • Collaborate with managers, LVN, and Director of Licensed Programs to evaluate the effectiveness of the program and implement improvements based on feedback and data analysis.

Consultation and Guidance

  • Act as the Infection Preventionist for all licensed programs in the absence of the LVN.
  • Provide support, consultation, and guidance to the LVN and staff to address complex medical issues.
  • Assist in the development and implementation of care policies and procedures.
  • Provide best practices and evidence-based guidelines with staff to improve participant care.
  • Provide solutions and guidance for resolving care-related challenges and issues.

Training and Education

  • Develop and assist with monthly staff trainings in healthcare conditions, illness prevention, lifting, and other assigned in-service trainings.
  • Assist with annual and/or remedial medication training for current staff and provide medication training to new hires within the first 30 days of hire in the absence of the LVN.
  • Collaborate with LVN and managers to identify, evaluate, and develop staff training needs.

Healthcare Plan Management

  • Develop individualized restricted healthcare and care plans tailored to the specific needs of each participant and follow all Title 22 regulations.
  • Oversee the implementation and completion of healthcare condition documentation (VNS, bowel and bladder, etc.) at each program regularly.
  • Track and monitor restricted healthcare and care plan reports to ensure they are current, and update as needed.
  • Collaborate with interdisciplinary teams to ensure comprehensive care for participants.

Communication and Coordination

  • Communicate any concerns or findings regarding healthcare concerns and/or evaluations with staff.
  • Respond to staff requests to assess and evaluate participant care concerns.
  • Facilitate communication with other medical professionals when necessary.
  • Foster a supportive and compassionate environment for participants, care providers, staff, and visitors.
  • Attend team meetings to provide care plan updates, introduce new practices, and/or preventive and healthy habits for participants when necessary.
  • Attend interdisciplinary team meetings (IDT) as the liaison in the absence of the LVN.

Documentation and Reporting

  • Collaborate with LVN, Program Manager, and Director of Licensed Programs to develop forms, procedures, protocols, and any other assigned documentation to support participants and/or staff.
  • Prepare compliance reports to regulatory agencies as required.
  • Oversee accurate and up-to-date documentation of all care plans, assessments, and evaluations.
  • Document and report any incidents, accidents, or unusual occurrences involving participants, following organizational protocols.
  • Oversee and maintain records to demonstrate compliance with Title 22 regulations and other relevant care standards.
  • Oversee internal and external audit documentation, ensuring all records are complete and accessible.
  • Document detailed progress notes on the health status and interventions provided to participants.
  • Maintain comprehensive records of all training, including attendance, topics covered, etc.
  • Retain detailed communication logs between Oparc staff, medical professionals, and care providers.
  • Document any updates or changes to participant care needs, ensuring all staff are informed and trained on new protocols.

Minimum Qualifications

  • Registered Nurse (RN), with a current CA license and knowledge of Nursing Scope of Practice.
  • Preferred experience working with individuals with intellectual and developmental disabilities (IDD) or in a similar caregiving, nursing, behavioral, or human services role. 
  • Superior oral and written communication skills to direct personnel. 
  • Possess outstanding leadership and organizational skills . 
  • Ability to provide comprehensive staff training, coaching, and supervision.
  • Excellent problem solving and conflict resolution skills with the ability to handle emergencies. 
  • Ability to pass a Department of Justice (DOJ) background check and fingerprint clearance, complete TB test, pre-employment physical, and provide proof of required immunizations (as applicable).  
  • Valid CA driver’s license, proof of current vehicle insurance, and must have a clear driving record with three (3) years of driving experience.
  • Reliable transportation to travel and attend off-site meetings.
  • CPR, First Aid, and Crisis Intervention (e.g., CPI) certifications — or ability to obtain upon hire.

Preferred Qualifications

  • Must possess and demonstrate an advanced level of proficiency in Microsoft Office, including Word, Excel and Outlook.
  • Bilingual (Spanish, ASL, etc.) is a plus.

Work Environment Requirements

  • Able to work in both indoor and community-based environments.
  • Willing to support individuals during community outings, job coaching, and/or volunteer activities.
  • Able to lift up to 25 pounds by self and up to 150 pounds with one other person.
  • Assist with physical transfers, if necessary.
  • Able to push a wheelchair with a seated participant up to one (1) mile.
  • Standing, walking, sitting, pushing, bending and pulling required.
  • Squatting and repetitive motion may be necessary.
  • Use chemicals in accordance with manufacturer’s labeled instructions.
  • Adhere to the proper care and responsible use of Oparc issued property, tools, and equipment.

 

Location: Oparc – ADC Monte Vista (assigned worksite may vary)

 

Reports To:   Director of Licensed Programs

 

Schedule: 8 hours per month. Days and hours may very based on the needs of the program.
