Optimus Healthcare is looking for an experienced RN with OB/GYN experience to join our primary care clinic in Bridgeport, CT.   Join our team-based model of care delivered in a kind and compassionate manner. This role has one late evening per week and participates in the Saturday rotation.

POSITION SUMMARY

The Registered Nurse coordinates the primary care services provided to patients from diverse populations in a professional manner. Works with the Practice Manager, Director of Nursing, Nurse Manager, and the medical providers to ensure compliance with Optimus’ standards of care. Provides clinical and administrative support and functions as a resource and mentor for new staff and students assigned to their work site. Assists the Director of Nursing and Nurse Manager in ensuring that nursing staff follow the Nursing Practice Act in their delivery of comprehensive patient care with maintenance of strict confidentiality.

The essential functions of the Registered Nurse include but are not limited to:

  1. Promote and restore patients' health by implementing the nursing process; collaborates with physicians and multidisciplinary team members; provide patients with physical and psychological support; supervise assigned team members.
  2. Promote patients’ independence by establishing patient care goals; teach and respond to patient’s questions regarding their medical condition, medications, and self-care skills.
  3. Ensure quality of care by adhering to therapeutic standards; measure health outcomes against patient care goals and standards; make or recommend necessary adjustments; follow nursing standards of care set by state board of nursing, state nurse practice act, and other governing agencies and regulating bodies such as DPH, CMS, HRSA and The Joint Commission
  4. At the direction of the Nurse Manager, take responsibility for facilitating the clinical orientation for new hire onboarding.
  5. At the direction of the Nurse Manager, provide performance feedback as needed for LPN and MA.
  6. Assist Nurse Manager in daily oversight of the LPN and MA.
  7. Report clinical and operational issues that need to be addressed within the facility to the Practice Manager, Director of Nursing or Nurse Manager as soon as possible for immediate response.
  8. Collaborate with the Practice Manager and Nurse Manager to keep clinical staff informed of any changes which need to be implemented.
  9. Work with providers to coordinate care for patients and to provide appropriate clinical support.
  10. Resolve patient concerns and/or complaints by utilizing multidisciplinary team strategies.
  11. Monitor all tracking systems for patient’s results and ensure that all referrals/images are processed and tracked per Optimus’ policy.
  12. Assist the Nurse Manager to oversee tasks of the clinical staff and its timely completion. 
  13. Protect patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations 
  14. Assist the Nurse Manager with maintenance of standard room inventory.
  15. At the direction of the Nurse Manager, take responsibility for submitting supply order for medical supplies.
  16. Maintain a safe and clean working environment by complying with procedures, rules, and regulations.  
  17. Delegate and monitor duties according to staffing and competencies. 
  18. Perform triage and direct patient care as needed.
  19. Assist the Nurse Manager to maintain and oversee the medication sample closet.  
  20. At the direction of the Nurse Manager, oversee the VFC Program per DPH protocols.  
  21. At the direction of the Nurse Manager, oversee the CIRTS Program and vaccines management for children birth to age 2 years.
  22. Participate in Morning Huddles in preparation for patient visits.
  23. Assist the Nurse Manager to ensure clinical staff is competent on Point of Care testing and procedures.
  24. Assist Nurse Manager with management of the emergency medication kit. 
  25. Remain current with new trends and philosophies in nursing care.
  26. Participate in committee meetings as needed.
  27. Perform other related duties as required and/or assigned. 

 ADDITIONAL GENERAL REQUIREMENTS

Professional positive attitude, vision, understanding of customer service principals, trustworthiness, and excellent interpersonal skills to successfully accomplish tasks necessary to meet high standards of ethical and social responsibility required by this position. Must possess a valid current State of Connecticut Registered Nurse license.  Requires the ability to drive to the satellites, to provide clinical coverage and to attend committee meetings.  

 JOB QUALIFICATIONS/REQUIREMENTS

  • Ability to plan and prioritize own work and that of others when needed.
  • Proficient in using EMR; knowledge of Microsoft Word, Excel and PPT

 EDUCATION:   Graduated from an accredited registered nursing program approved by the State of Connecticut Department of Public Health with an Associate’s Degree; Bachelor’s Degree preferred

 EXPERIENCE:   Minimum of one to two years of experience as an RN or prior experience as an LPN in a community health setting.  Familiarity with tele-medicine and prior experience a plus.

 LANGUAGE SKILLS: Bi-lingual English / Spanish is helpful; demonstrate the ability to effectively translate medical terminology as assessed by successful completion of language proficiency.

 MATHEMATICAL SKILLS: Basic mathematical skills as it relates to medication administration.

 REASONING ABILITY:   Critical thinking skills, ability to create positive motivation.  Strong verbal and written communication skills.  Ability to work independently. Must have the ability to organize and manage multiple priorities and work under time pressure deadline. Ability to demonstrate consistently sound judgment, accessibility, and responsiveness to all team members.  Self-starter who proactively and continually identifies more efficient processes and procedures.

 LICENSURE / CERTIFICATION:  Valid and current RN License to practice in the State of Connecticut.  CPR certification required upon hire and recertification bi-annually.
