Position Summary
The Community Care Navigator position is temporary, full-time, 40 hours per week (non-exempt, benefits eligible) and will take place at all our locations. The Community Care Navigator will work to bridge the gap between communities and resources by assessing needs, identifying barriers, and connecting individuals to resources through case-management, crisis-intervention, support, and advocacy. The Community Care Navigator will assist the Director of Donations and Resources as needed. Some weekends and some on-call in response to crisis situations will be required. Flexible schedule required. Will require some local travel (reimbursable). This position is in-person.
Job Responsibilities
- Provides support and advocacy to families through home visits, phone calls, criminal justice advocacy, and referrals to other support organizations.
- Assures coordination and facilitation of educational forums, family and group support sessions, trainings and other gatherings by Community Care Navigators.
- Develops relationships with partner organizations and builds “warm referral” network for linkage.
- Assists data team with data collection and required reporting.
- Assists Community Care Navigators in accessing resources such as crisis response, food delivery, emergency supplies, etc.
- Works with Community Care Navigators to identify and address safety concerns and create a plan for long term safety and well-being.
- Documents detailed client case-notes and other items as assigned.
- Maintains accuracy of budget and spending plan.
- Participates in all required evaluations and audits.
- Demonstrates New Life Centers’ values.
- Attends required meetings and trainings.
- Driving to various locations required.
- Required to work on the warehouse, food pantry/food handling, and respond to crisis situations related to emergency situations in the Chicagoland area.
- General manual labor duties such as performing physical tasks like lifting, moving, stacking, organizing, carrying materials, and maintaining a clean and save worksite.
- Other duties as assigned by the Supervisor.
Skills & Qualifications
- Must be able to drive (25 years of age for NLC insurance).
- Must be able to drive a NLC cargo/liftgate truck.
- Must be able/willing to travel locally for deliveries etc.
- Must be available to respond to crisis outside of regular scheduled hours when assigned.
- Must become a Certified Mandated Reporter. Training provided.
- Must pass a background check, must not have any pending cases.
- Must pass a Child Abuse and Neglect Tracking System background check.
- Must pass a National Sex Offender Registry background check.
- Must have a working phone and phone number.
- One year experience working in the human services field in a capacity that involves direct services to at-risk youth.
- Cross-cultural competency.
- Ability to communicate effectively both written and verbally.
- Proficient in Microsoft Word and Excel.
- Must be willing to be trained and proficiently utilize database(s) and information tracking systems relevant to duties.
- Must keep confidentiality of participant information.
- Ability to establish and maintain effective working relationships with others.
- Valid Illinois driver’s license, insurance, good driving record, access to a vehicle. Subject to driving background check.
- Bilingual English/Spanish required.
