This position works closely with the Quality and Analytics teams and helps integrate clinical programs, network saving initiatives, quality initiatives, and resources into the practice workflows with the intent to optimize the patient experience and positively impact physician engagement and quality of care provided.%3C/p%3E%3Cp%3E%3Cstrong%3EExceptional Skills and Qualifications%3C/strong%3E%3Cbr%3EApplicants for this position should be able to communicate effectively with internal team members, and key stakeholders – including public speaking, PowerPoint, dashboards, visualizations, and/or other presentation formats. Applicants must be highly organized and self-motivated individuals with the ability to adapt to various workspaces and work autonomously. This applicant has a collaborative work style with the ability to work across different teams, areas of expertise, and adapt to ambiguous environments and clientele.%26nbsp;%3Cbr%3E%26nbsp;%3C/p%3E%3Cul%3E%3Cli%3EBachelor’s degree or equivalent work experience required.%3Cbr%3E%26nbsp;%3C/li%3E%3Cli%3E5 or more years of leadership experience in practice management. Provider relations, and project management.%26nbsp;%3Cbr%3E%26nbsp;%3C/li%3E%3Cli%3E%3Cp%3E3 or more years of experience in practice re-design work, including Patient Centered Medical Home, Practice Transformation, Quality Improvement,%26nbsp;%3C/p%3E%3Cp%3E%26nbsp;%3C/p%3E%3Cp%3EACO development, Ambulatory Care, Quality and Efficiency metrics, and Electronic Medical Records.%3Cbr%3E%26nbsp;%3C/p%3E%3C/li%3E%3Cli%3EHealth plan experience with a provider perspective and strong orientation to value-based care principles and the health policy landscape.%3Cbr%3E%26nbsp;%3C/li%3E%3Cli%3EExperience in data analytics comprehension.%26nbsp;%3Cbr%3E%26nbsp;%3C/li%3E%3Cli%3EMust have a valid Driver’s license and the ability to travel regionally up to 50% of the work week.%3Cbr%3E%26nbsp;%3C/li%3E%3Cli%3EProficient in Microsoft Office products (Excel and PowerPoint).%3Cbr%3E%26nbsp;%3C/li%3E%3Cli%3EHypothesis-driven to identify trends, predict issues, highlight critical areas, and develop corrective action plans.%26nbsp;%3Cbr%3E%26nbsp;%3C/li%3E%3C/ul%3E
