Role: Hospice & Home Health Community Liaison
- Location/Type: Naperville & West Suburbs • Onsite with field travel
- Pay: $70K–$85K base • $90K OTE • Uncapped commissions (earn $100K+)
- Schedule: Mon–Fri • 8am–5pm • Rare overnight travel (≤1x per quarter)
What you’ll do
Grow hospice and home health admissions with clear, reachable goals
Host patient/family info sessions to cut confusion and build trust
Hand off referrals fast to clinical teams (no bottlenecks, no lost admits)
Track and report results in CRM/EHR (clean tools, not busywork)
Educate the community on care options that actually help patients
Support paperwork and signatures only when needed—not dumped on you
Must-haves
1+ year hospice or home health marketing experience
Active driver’s license and reliable insured vehicle
Clear driving record
Strong written and verbal communication skills
Comfort with Microsoft Office and EHR systems
Perks & pay
Pay: $70K–$85K base • $90K OTE • Uncapped commissions

Mileage reimbursement at $0.70/mile
Bonus: $1,500/month at goal + $150 per referral above

Mon–Fri • 8am–5pm
Field-based in Naperville/west suburbs
Travel ≤1 overnight per quarter
