Location: Rogers Park or Chatham
The incoming Front Door Diversion Program (FDDP) Administrator will be responsible for managing incoming FDDP referrals from Maximus. The Program Administrator will support program compliance with grant contract performance measures and timelines. They will collaborate with Trilogy FDDP intake assessors and FDDP management to ensure smooth client transition from hospital to temp housing. The Program Administrator will also provide support to the housing administration team and Clinical Director of Housing. This position will support communication between teams and with IDHS-DBHR. This position will ensure compliance to data and reporting according to the IDHS grant requirements. Program Administrator will directly supervise admin and program assistants within the department.
Responsibilities:
- Ensure timely and seamless flow of FDDP referrals from Maximus to internal Trilogy Intake staff assigned to FDDP
- Document and report on any referral issues to state partners and leadership as needed.
- Report on trends, community needs, and team updates to Supervisor.
- Manage weekly, monthly, and quarterly reporting to UIC and IDHS-DBHR.
- Support leadership team with tracking caseload needs, monitoring grant progress and client milestones, and ensuring program compliance.
- Provide direct supervision and oversight to program assistants and administrative assistants.
- Communicate and collaborate effectively with program leaders, state or other external partners, etc. and problem solve barriers and bottlenecks to support efficient linkage to care.
- Track updates and progress in internal systems, as well as state monitored systems such as Caspio and AssessmentPro.
- Facilitate direct and consistent communication between administrative teams and clinical teams to support collaboration and efficient workflows.
- Identify opportunities for consolidation, cross training, and shared responsibilities to ensure teams have consistent high-level coverage for administrative tasks.
- Support clinical teams with linkage to Statewide Referral network for clients, and support ongoing efforts to transition clients towards alternative permanent housing pathways.
- Facilitate quarterly contact with partner hospitals.
- Support with data entry, grant compliance, and administrative duties as needed for Permanent Supportive Housing programs.
- Represent Trilogy in a positive and professional manner; and communicate effectively and professionally to others about potential organizational barriers/issues.
- Perform other related duties and/or projects and participate in committees/workgroups as assigned.
Qualifications
- Associate’s or Bachelor’s degree in any field.
- Demonstrated work experience in an administrative, clerical, or site management position
- Thorough knowledge of Microsoft applications (Outlook, Teams, Word, Excel, etc.), report generation, and database management tools
- Excellent customer service and organization skills
- Prior experience in mental health field preferred
