Position: Community Health Worker 

Department: Behavioral Health Services

Location: l575 Blue Hill Avenue, Mattapan, MA 02126

Reports To: Social Services Manager, Behavioral Health


Job Summary:


The Community Health Worker position is grant-funded in partnership with the Boston Public Health Commission and is part of an initiative to create a network of Community Healing Response Network (CHRN) within the City of Boston. The CHRN provides comprehensive trauma support to residents of the City who are impacted by community violence. The CHW works in close collaboration with our Trauma Recovery Clinician and others on our team to provide recovery services and support to individuals and families.


Under the direction of the Social Services Manager within the Behavioral Health Department, the Community Health Worker (CHW) responsibilities include: outreach and engagement to those impacted by community violence, case management, assistance with supports and service linkage; participation in briefing calls; conducting home visits as needed in partnership with other team members; ensuring active, accurate and consistent communication with other staff and partners; timely and consistent completion of all necessary documentation; and participation in training and supervision. The CHW may function as a compassionate presence at community meetings, funerals, or events and conduct outreach to families and other agencies in the community affected by traumatic events.


In addition, the incumbent is required to follow all other protocols established by Mattapan Community Health Center, all other appropriate standards of practice, licensing and certification and other regulatory agencies requirements.  


RESPONSIBILITIES

  1.    80% Community Care and Outreach  
  • Provide strength based, culturally competent outreach, education, engagement, family/peer support and service linkage to clients and their families impacted by trauma. Services include support enhancing daily living skills, coordinating services, meeting basic needs (shelter, food, healthcare, etc.), developing crisis plans, and fostering empowerment and recovery.
  • Coordinate behavioral and social support services within client’s natural environment (homes schools, community)
  • Provide response services in the community after an incident of violence that may include community canvassing, organizing coping support groups, being a compassionate presence at community meetings, psychological first aid, etc.
  • Facilitate prevention and promotion activities such as curriculum-based parent groups, youth dialogues, workshops onetime events, etc. 
  • Conduct outreach to community-based partners, offering resources and training around preventing and reducing the impact of violence.
  • Provide regular documentation of activities in conjunction with CHRN protocols and expectations.
  • Participate in trainings and supervision for CHRN and the health center at large.
  • Perform other tasks as required.


  1. 20%  Program Planning, Development, and Implementation
  • Collaborate with other health center staff, consultants, and community partners, as appropriate.
  • Work in close conjunction with Community Partner, Mothers for Justice and Equality for outreach and other community engagement efforts following an incident and beyond.
  • Provide in-service and community presentations and education to staff, parents or community organizations on trauma response and resiliency as needed.

Perform other related duties as required or assigned by the Social Services Manager or Director of Behavioral Health Services. 

Requirements

Strong ability to connect and partner with culturally diverse individuals and families; experience in the field of trauma support; ability to work effectively with others, experience with care coordination; facilitation of prevention and promotion activities; training or experience with community outreach and organizing with at least 1 year experience addressing issues of violence and trauma. Possess strong organizational, communication, time management, and writing skills. Ability and willingness to work on a flexible schedule to meet the needs of the community and families. Ability to complete necessary documentation in a timely manner. PC literate with a knowledge of MS Word, PowerPoint, Excel, and Internet.


 

BACKGROUND REQUIRED:

H.S. Diploma/GED required with a minimum of 3 years of community-based experience in trauma and/or family support services. Associate degree preferred in Human Services field with 1-3 years related work experience preferred. Experience working with low-income and multi-cultural communities, sensitivity to socio-economic and psychosocial issues impacting communities of color. Haitian Creole or Spanish language capacity strongly preferred.


This is a full- time position Monday to Friday with some evenings and some Saturdays.
