Description

Position Summary:

Under general supervision, engages with community partners, builds community relationships, and offers support to the community on behalf of Advantage. Assists with referrals to the agency and attends interdisciplinary meetings as needed. Performs psychosocial assessments, provides counseling/therapy to clients in the form of individual or group services.

 

Job Responsibilities and Performance Standards:

  1. Acts as community liaison and client advocate to coordinate services.
    • Develops and maintains a network of community contacts.
    • Participates in strategy or review sessions, committees and task force work to facilitate linkages and communication.
    • Represents the facility in planned community meetings.
    • Participates in meetings pertaining to case management, case collaboration and discharge planning providing appropriate patient/consumer information.
    • Arranges and/or provides follow-up appointments.
    • Makes referrals to appropriate agencies for support services.
  2. Serves as organization resource in assigned areas.
    • Communicates accurately and clearly with the patient/client/consumer and his/her family members/significant others regarding identified treatment goals.
    • Monitors patient's/client's/consumer's progress through interviews, discussion with team members, chart reviews or other methods. Accurately conveys progress information to family members/significant others as appropriate.
    • Writes concise, accurate, and timely progress notes in the patient's/client's/consumer's chart as required by policy.
  3. Collaborates/communicates with Director of Access and Routine Care to ensure clinical services at the Morgan County location are adequate, appropriate, and accessible.
    • Remains knowledgeable about guidelines and requirements that regulate provision of Routine Care, including, but not limited to: DBHDD Utilization Guidelines, CARF, ASO requirements, DBHDD contractual obligations, DATEP/HFR regulations, Advantage policies and procedures, etc.
    • Ensures protocols are in place to maximize access to services. Collaborates with the Director of Access and Routine Care and the Director of Youth Services, and other team members to address and problem solve barriers to access.
    • Provides 20 hours of direct service each week through assessments, individual, family, and/or group counseling.
    • Maintains a positive team-oriented work culture.
    • At the direction of the Director of Access and Routine Care and the Director of Youth Services, collects data on access to services, treatment outcomes, and client experience.
  4. Ensures facility maintains professional, welcoming, accessible and safe environment
    • Maintains accurate inventory and replacement of all equipment and furnishings used in the program.
    • Maintains facility in accordance with local/state laws and established agency policies.
    • Conducts facility maintenance reviews to ensure compliance with established guidelines and takes corrective action as needed.
    • Ensure routine and emergency maintenance is performed on all equipment to keep in operable condition.
    • Ensures physical structures/facilities are appropriate to model of services provided and meet health, safety, and accessibility standards.
  5. Promotes a supportive team environment that represents the mission, vision, and values of Advantage and creates a collaborative team approach to problem solving and planning.
    • Attends agency and external meeting and training, as assigned.
    • Other duties as assigned.

Qualifications


Minimum Qualifications:

  • Master’s Degree with full independent license (LPC, LMFT, LCSW).
  • A commitment and dedication to working with adults and youth with severe and persistent mental illness.
  • Organized and detail oriented.
  • Strong clinical skills (including knowledge of diagnostic impressions/criteria, crisis interventions, individual and group therapy techniques, various therapeutic models for treatment, etc.)
  • Strong time management skills.
  • Strong critical thinking and problem-solving skills.
  • Moderate to high proficiency with Microsoft Office (Excel, Word, Outlook, etc.).

 

Certificates, Licenses, Registrations:

Full licensure as LPC, LMFT, or LCSW.
