Position Overview:
The Center Coordinator plays a vital role in ensuring participant programming is delivered. As Center Coordinator, your role will involve scheduling and coordinating all 1:1 and group participant sessions for the care team, dynamically updating schedules, and answering phone calls from participants and community stakeholders.

Key Responsibilities - your role will involve:

Scheduling 1:1 and group sessions between participants and the care team:
  • Managing a master calendar of participant 1:1 and group sessions within Concordance’s scheduling software 
  • Adapting schedules daily to account for community appointments, participant and team member availability, and participant needs  
Communicating with participants and Concordance’s multi-disciplinary care team:
  • Collaborating with team members to ensure participant appointments are scheduled to meet their needs, including triaging urgent needs  
  • Communicating upcoming schedules for 1:1 and group sessions to participants and team members
  • Directing, coordinating, and resolving ad hoc participant questions
  • Confirming scheduled sessions and event attendance 
Collaborating with the multidisciplinary care team and community:
  • Connecting participants and their families with community faith organizations 
  • Documenting sessions (for individuals and groups), including noteworthy participant interactions 
  • Participating in internal meetings (Behavioral Health and Wellness team meetings, All Team meetings, 1:1 supervisor meetings, and training sessions) 
  • Occasionally, help set up for Center events or move classrooms around, including but not limited to carrying and setting up tables and chairs. 

The right candidate will come with the following attributes, education, experience and meet the physical requirements.

PERSONAL ATTRIBUTES:
  • Passion for our mission 
  • Knowledge of Microsoft Word, Excel, and scheduling software  
  • Detail-oriented and organizational skills 
  • Skill in managing multiple schedules, resolving schedule conflicts, and adapting to dynamic circumstances 
  • Collaborative attitude and excitement about working in a multidisciplinary environment 
  • Effective communication skills with both internal and external stakeholders  
  • Commitment to professionalism, integrity, and compassion in all interactions 
REQUIRED EDUCATION AND LICENSING:
  • Bachelor’s degree or equivalent work experience 
  • CPR/adult first aid certification preferred 
SKILLS AND COMPETENCIES:
  • Experience working with vulnerable populations preferred
  • Demonstrated ability to manage detailed schedules and collaborate with others in a fast-paced, team-focused environment
