Specific Responsibilities
• Working closely with the Family Support Manager and Service Coordination Manager to plan
and implement strategies for outreach and coordination of services with communications with
collaborative partners
• Represent Good Samaritan and participate in provider meetings in person or virtually with
community partners to build new connections to expand support for families
• Provide case management/family advocacy to 20-25 families per year, or as agreed upon with
supervisor, working with each family for 3-18 months or participants with families who have
immediate or short-term needs
• Conduct intake, assessment, and service plan development collaboratively with families
• Assist families in identifying their own needs and strengths and to set and reach their own
goals
• Coordinate and initiate referrals between Good Samaritan FRC, Bridges Clinic, and other
relevant community resources; monitor progress of referrals and actively engage with clients in
the effort to support the goal of successful and sustained engagement of services.
• Teach and model skills and strategies for accessing services, goal setting, problem solving,
conflict resolution, communication, parenting, and other areas relevant to family’s well-being
• Attend offsite meetings and accompany families to their appointments and hearings as
necessary to offer emotional support and translation assistance
• Facilitate, co-facilitate, coordinate and/or assist in person or virtually with workshops,
curriculum-based classes, support groups and/or other activities for families
• Explain Good Samaritan programs to community members, service providers and new
participants
• In coordination with team, conduct outreach and participant recruitment for each assigned
program activity
• Work with Mental Health Consultant when participants require further intervention and/or
referrals
• Maintain intakes, assessments, case notes, files, and other documentation and data for each
program activity
• Meet weekly with supervisor as scheduled
• Attend mental health consultation and other meetings as scheduled
• Participate in a variety of case-related meetings at family, agency and multi-agency levels
• Participate in training opportunities which are deemed beneficial to professional growth and
position
• Work closely with agency staff and Bridges Clinic team to ensure referrals, services, and
resources are fully integrated within the cross-sector collaboration.
• Comply with the legal standards for mandated reporting
• Ensure that programs and services are aligned with Good Sam’s Mission and Vision

Minimum Qualifications
• BA in Social Work/related field or equivalent years of experience
• Minimum of one year experience in case management or related case work
• Experience in client interviewing, assessment, and service plan development
• Experience working with Latino immigrant population and understanding of immigrant issues
• Experience with facilitation of workshops, classes and/or support groups
• Excellent and careful attention to detail
• Ability to work in a fast‐paced, high need environment
• Must possess good organizational and time-management skills
• Demonstrate good judgment and have excellent problem-solving abilities
• Always maintain a professional demeanor and always maintain confidentiality
• Must be able to work evenings and weekends as needed
• Must be self-motivated, have excellent work ethic and be able to work independently
• Proficient in Outlook Suite (Word, Mail, Excel, etc.)
Desired Qualifications
• Experience working in a Family Resource Center
• Education or training in Child, Adolescent and/or Family Development, or related field
• Certification or experience in the facilitation of parenting classes or other evidence-based
curriculums
• Experience designing/modifying curriculum for support groups, child development, child oral
health workshops and/or infant massage classes
• Education, training, and/or experience in public health or clinical care; relevant certification
preferred.
• Ability to work independently and as part of a team
• Data Entry experience (Salesforce, CMS platforms)
Requirements
• Must clear full background check (Department of Justice, FBI, and Child Abuse Index)
• Candidates must go through a background check after a conditional offer is made.
• Must have TB clearance
• Bilingual (Spanish and English; reading/writing/speaking)
• Travel required (accompaniment to appointments)
