Description
- Provide services to at-risk youth referrals from Cook County Juvenile Probation, Illinois Department of Juvenile Justice, Chicago Juvenile Intervention Support Center, as well as local school and community-based partners and families from participants.
- Provide case management services that include, but are not limited; individual needs assessments, goal setting, face-to-face and phone conversation on a weekly/bi-weekly basis, family engagement, linkage with community agencies, and follow-up services.
- Maintain an active caseload.
- Work with each youth to develop an Action Plan.
- Link at-risk youth with a community-based services.
- Coordinate outreach and engagement.
- Assist youth to determine what factors put them at risk of engaging in or becoming victims of aggressive behavior and other anxieties.
- Determine what methods each youth should use in order to avoid risk-taking factors and reach their goals.
- Develop and maintain partnerships within the targeted communities to cultivate relationships with local community resources and strengthen referral base.
- Re-engage youth that have inconsistent attendance, work with families to bring resources to the home, and connect with programs.
- Enter case notes, assessments, and case plans in a timely manner as well as respond to e-mail communication, and all other required information.
- Complete required reports and data material for SGA and DHS/DMH.
- Perform other duties as assigned.
Requirements
Education:
- Bachelor’s Degree in psychology, social work or behavioral science field.
Experience:
- One to three years of experience working one-on-one with youth involved in the juvenile justice system.
- Bilingual/bicultural in Spanish/English, preferred.
- Experience working with teens and young adults, knowledge of learning styles and ability to engage high risk youth and their families.
- Experience working in neighborhoods on Chicago’s south and west sides
- Knowledge of Cook County Juvenile Justice System.
- Experience working with individuals dealing with factors that put them at-risk of becoming victims of aggressive behavior.
Other:
- Sensitivity to cultural diversity is required. Ability to communicate and cooperate with diverse families, various professionals and community groups.
- Must be able to make community and home visits to clients.
- Exercises discretion and independent judgment in executing tasks.
- Must demonstrate the ability to establish helping relationships with families of varying educational, experiential and socio-economic backgrounds and handle emergency and crisis situations.
Key Competencies:
- Competent in Microsoft Office tools and inputting services in an online database.
- Valid Driver’s License and Automobile Insurance.
Physical Requirements:
- Must be able to remain stationary 50% of the time.
- Must be able to frequently move inside the office to access file cabinets, office machinery, etc.
- Must be able to transport oneself to and from home visits.
- Occasionally ascends/descends stairs.
- Constantly moves to assist during program activities.
- Frequently moves desks, chairs, tables and easels.
- At times may be required to lift up to 25 lbs. for various classrooms and event’s needs.
- Frequently moves in outdoor weather conditions.
