Winona Health is a registered Community Health Worker Apprenticeship site through the Minnesota Department of Labor and Industry. This is an opportunity for those interested in a Community Health Worker Training Program (CHWTP) Apprenticeship, to be eligible to receive an additional short-term stipend in addition to wages.

Position Overview:

The Community Health Worker employs community-based care coordination strategies outlined in the Pathways Community HUB model to improve health outcomes for targeted populations. Through community-based care coordination, this position will provide support to participants as they navigate the health care and social service systems in the Winona area. This position aims to increase access to support services, improve the quality and cultural competence of their care, and increase the health knowledge and self-management skills of participants served. The Community Health Worker has a deep knowledge and understanding of the Winona-area community, and holds lived experience that uniquely equips them to bridge barriers related to culture, race, language, socioeconomic status, or other factors. This position requires extensive travelling throughout the community working with varied populations, oftentimes in participant homes. This position works closely with other Winona Health caregivers, area social service organizations and other partners.

Essential Duties & Responsibilities:

  • The Community Health Worker will serve as a critical linkage between health and social services agencies by providing compassionate and professional outreach to participants identified at Winona Health or partner agencies.
  • The Community Health Worker will follow up with participants in their homes or other community-based locations to conduct an in-depth evaluation of their social, medical, employment, housing, financial, or other social needs.
  • The Community Health Worker will apply their shared life experience and training to assist participants with education, advocacy, care coordination, coaching, and navigation to ensure these needs are systematically addressed.
  • The Community Health Worker provides communication support and acts as a participant advocate on issues of access and use of medical, social, or other services.
  • As such, the Community Health Worker may facilitate transportation for participants to access healthcare or social service appointments, may attend appointments with participants, facilitate paperwork completion, ensure instructions are understood, and that progress towards self-sufficiency is being achieved.
  • The Community Health Worker ensures the privacy and security of PHI (Protected Health Information) as outlined in Winona Health's policies and procedures relating to HIPAA compliance.
  • Participant interactions are documented in a timely and thorough manner using a HIPPA compliant electronic data collection platform.
  • The Community Health Worker will attend meetings and trainings as needed.
  • Overnight travel to conferences or community health worker trainings may be required on occasion.
  • The Community Health Worker may be required to distribute and discuss health promotion information to community groups as needed.
  • Other job duties as assigned.

Skills and Experience:

Required:

  • High School Diploma or Equivalent
  • Community Health Worker (CHW) certificate completion
  • Minimum one-year experience volunteering or working in human services, healthcare, community outreach, health education, or related direct service
  • Understanding of various healthy coping skills
  • Knowledge of local community agencies and resources
  • Intermediate computer skills: Word, Excel, Outlook, and PowerPoint
  • Ability to communicate effectively orally and in writing
  • Passion for working in the community to address issues of inequity
  • Strong social and listening skills are desired, including the ability to build trusting relationships and work effectively with socially and medically complex participants in a variety of non-traditional settings
  • Empathy, patience, and persistence are critical skills
  • Ability to communicate collaboratively in a team while managing multiple priorities is required
  • Creative problem-solving and attention to detail
  • Effective time management skills, and sound judgment while keeping professional boundaries
  • Valid driver's license, valid auto insurance, good driving record and access to reliable personal transportation

        Internal Applicant Policy:
