Basic Function: Under the direction of the Health Services Supervisor, the Health and Services Coordinator assume primary responsibility for the medical appointments and for ECSS Residential individuals.

Duties/Responsibilities:
ESSENTIAL DUTIES AND RESPONSIBILITIES:
The list of essential duties and responsibilities, as outlined herein, is intended to be representative of the task to be performed. The omission of an essential function does not preclude management from assigning duties not listed herein is such duties are a logical assignment to the position.

1. Ensure delivery and/or coordination of all Residential services are in compliance with DHS Rule 115 & 116, CILA & ICF, agency mission, agency policy & procedure, program guidelines and best practice.

2. Completes documentation as required by DHS rules and regulations and ADA S. McKinley policies and procedures.

3. Assists with scheduling and transportation for medical/dental appointments. Ensure procedures to promote optimum health care and behavior supports are followed.

4. Advocates for, promotes, and protects the rights of all individuals.

5. Proper documentation of all medical visits in Envolv

6. Keeps abreast of applicable laws, rules/regulations, and policies and procedures promulgated through code agencies responsible for surveillance and regulatory oversight of ADA S. McKinley’s residential services, including IDPH, DHS, and any other entity to which the adult residential programs maintain accountability.

7. Assures that the services provided by the agency function in a professional and ethical manner, and that such services operate in compliance with licensing regulations and applicable laws.

8. Collects and reviews medical information needed for the completion of monthly reports.

9. Comply with all audit systems in place, including reviewing and tracking of laboratory reports.

10. Maintains annual American Heart Association certification in First Aid and CPR.

11. Successfully completes ADA S. McKinley required training, re-training, and any additional training as required by ADA S. McKinley policies and practices, or as assigned by his or her immediate supervisor.

12. Other duties as assigned by supervisor.

13. Schedule, accompany/drive participants to medical appointments for all Ada residential programs: approximately 3-4 a day as directed.

14. Facilitate tracking and ordering labs while following medication orders as prescribed by the physician.

Qualifications:

Education: This position requires minimum of High School Diploma/associate’s degree (A.A.) preferred.

Experience: Minimum of two years of supervisory experience and cumulative experience with increasing responsibility within a human services or social services working with individuals with development disabilities.

Skill, Knowledge, and Abilities:
The incumbent must be able to effectively communicate orally and in written with internal/external vendors/clients, staff and management, and maintain professional relationships. This position requires the incumbent to be on-call after normal business hours to address any emergencies and/or concerns.
This position must possess valid/current driver’s license and annual clearance of the Agency background screening process.

Mental/Physical Demands: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Ability to read and interpret documents such as instructional materials, operating instructions, books, catalogues, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization. While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to stand, walk, and sit.
The employee is occasionally required to use hands to finger, handle, or feel, and reach with hands and arms. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

Environmental Conditions: The noise level in the work environment is usually moderate. Work is performed in a residential or classroom environment.
