We are looking for a passionate Clinical Coordinator (RN) to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving


Position Summary: The Clinical Coordinator (CC) serves as a Homecare Homebase (HCHB) superuser and back-office support for clinical operations. The CC ensures accurate workflow completion, medical coding, and quality assurance, while collaborating with leadership to maintain compliance and support patient care. The role also includes performance audits, education support, and acting as a resource for the clinical team. 

 Essential Duties:

  • Complete time-sensitive workflow in HCHB; provide support across programs as needed.
  • Serve as an HCHB superuser and trainer in partnership with the Clinical Educator.
  • Validate medical coding accuracy by Medicare/Medicaid guidelines; ensure corrections are made prior to claim submission.
  • Conducts medical records audits are assigned.
  • Collaborate with leadership to ensure clinical documentation support claims submission; notify QA Manager when claims need to be held or delayed for processing.
  • Communicate trends and deficiencies promptly; assist in developing corrective actions and improvement plans.
  • Compile documentation to support Quality Assurance Performance Improvement (QAPI).
  • Ensure test results (labs, etc.) are received and reported to appropriate staff.
  • Verify Patient Notice of Non-Covered Items is documented per regulatory requirements.
  • Conduct ongoing medical record reviews for compliance with state, federal, and accreditation standards.
  • Monitor EMR processes to ensure data integrity and operational alignment.
  • Perform other duties as assigned.

Qualifications:

  • Graduate from an accredited school of nursing, minimum associate’s degree in nursing; Bachelor’s degree in nursing preferred.
  • Active Registered Nurse (RN) license.
  • Minimum of 2 years of hospice clinical experience.
  • Minimum 1 year managing EMR back-office workflows; HCHB experience preferred.
  • Valid driver’s license, auto insurance and reliable transportation.
  • Strong knowledge of state/federal regulations and hospice care standards, ACHC or CHAP knowledge preferred.
  • Strong observation, critical thinking and decision-making skills.
  • Knowledge of hospice care, managed care, and applicable laws/regulations.
  • Skilled in ICD-10 coding.
  • Accurate documentation and auditing experience.
  • Experience in identifying and developing performance improvement initiatives. 
  • Detailed oriented, resourceful, and able to manage multiple priorities.
  • Strong interpersonal, oral, and written communication skills.
  • Ability to manage multiple projects independently with attention to detail.
  • Proficient in MS Office Suite (Word, PowerPoint, Excel, Outlook).
