Job Summary:
Provide comprehensive psychosocial assessments, interventions, and discharge planning services to patients and families while actively participating in hospital quality improvement initiatives. This dual-role position combines direct patient care with data analysis and process improvement activities to enhance patient outcomes and satisfaction.
Essential Functions:
Clinical Social Work Responsibilities:
- Conduct comprehensive psychosocial assessments to identify patient and family needs, barriers to care, and discharge planning requirements
- Provide crisis intervention, counseling, and emotional support to patients and families facing acute medical situations
- Collaborate with interdisciplinary teams to develop and implement treatment plans
- Facilitate family meetings and care conferences
- Coordinate discharge planning, including placement in appropriate care facilities, home health services, and community resources
- Provide education and support regarding advance directives and end-of-life planning
- Assess for abuse, neglect, or safety concerns and initiate appropriate interventions
- Maintain accurate, timely documentation in compliance with regulatory standards
Quality Initiative Responsibilities:
- Participate in quality improvement projects focused on patient experience, readmission reduction, and care transitions
- Collect, analyze, and report data related to social work interventions and patient outcomes
- Identify trends and patterns in discharge delays, patient satisfaction scores, and care coordination challenges
- Collaborate with quality department to develop and implement process improvements
- Participate in root cause analysis for quality events involving psychosocial factors
- Support development of evidence-based protocols for social work practice
- Assist with regulatory survey preparation and compliance monitoring
- Contribute to interdisciplinary quality committees and patient safety initiatives
Requirements
Required Qualifications:
- Master's degree in Social Work from CSWE-accredited program
- Current state licensure (LCSW, LMSW, or equivalent)
- Minimum 2 years of healthcare social work experience
- Knowledge of healthcare regulations and accreditation standards
- Strong analytical and data interpretation skills
- Proficiency in electronic health records and quality management software
Preferred Qualifications:
- Previous experience in quality improvement or process improvement initiatives
- Lean Six Sigma or similar quality methodology certification
- Experience with discharge planning and care transitions
- Knowledge of HCAHPS, CMS quality measures, and value-based care concepts
