As a Licensed Clinical Intake Coordinator, you’ll be the first clinical touchpoint for new patients—conducting assessments that inform life-saving care plans and guide individuals on the path to recovery.
The Role:
In this role, you’ll perform comprehensive biopsychosocial assessments to determine appropriate treatment plans for individuals with substance use and co-occurring mental health disorders. You’ll collaborate closely with the multidisciplinary team to ensure that each patient’s unique risks, strengths, and barriers are clearly communicated and considered throughout their recovery journey.
What You’ll Do:

Conduct diagnostic assessments and clinical interviews to evaluate mental health and substance use history

Develop detailed intake documentation and treatment recommendations based on individual needs

Identify barriers to treatment engagement and recommend recovery resources and support

Participate in interdisciplinary treatment planning and care coordination

Collaborate with medical providers, care coordinators, and peer support specialists

Provide crisis intervention services when appropriate

Maintain accurate clinical records and uphold compliance with regulatory standards

Contribute to team meetings and ongoing quality improvement efforts

Ensure timely and effective communication throughout the patient onboarding process
What We’re Looking For:
-
Master’s degree in Counseling, Social Work, Addiction, Marriage and Family Therapy, or a related behavioral health field

Completion of required post-degree supervision; minimum two years of experience preferred

Current and unrestricted clinical licensure to practice independently in your state (e.g. LCADC, LCSW, LPCC, LMFT, or equivalent)

Experience in substance use disorder assessment and treatment preferred

Knowledge of harm reduction, relapse models, and evidence-based practices

Strong documentation, communication, and assessment skills

Ability to work effectively both independently and within a team-based care model
