Facilitate and implement core programs as delegated by the program model for each property. Provide leadership, oversight, and coaching to Healthcare Coordination Specialists. Develop strategies to link residents with health care and care coordination services. Develop relationships with the primary care clinics, medical providers, care coordinators, health plans and other health service providers to increase the health and wellness of residents and reduce unnecessary healthcare utilization. This is a grant-based position. This position is eligible for a hybrid work schedule.

Duties
- Recruits, motivates, and manages Healthcare Coordination staff and volunteers to support Mercy Housing’s health and housing initiatives.
- Recommend, develop and/or provide related training and staff development resources for staff and third-party providers.
- Oversee staff’s navigation of health-related programs and behavioral health organizations.
- Maintain detailed knowledge of the health care system, key service providers and health care delivery system developments as related to the health needs of residents.
- Collaborate with Resident Service Coordinators (RSC) to identify residents most in need of care coordination and self-management assistance and determine what staff member is most appropriate to provide this assistance.
- Accurately identify/assess resident assets and needs in order to connect them effectively to resources and help them set personal goals; while recognizing their knowledge and experience.
- Develop partnerships with the primary care clinics, medical providers, hospital discharge planning team, care coordinators, health plans and other health service providers who work with the largest numbers of residents at high risk of health problems and elevated health care utilization (“high health risk”).
Minimum Qualifications
- High School Diploma or equivalent.
- Three (3) years of experience in a position that involved coordination among frequent interaction with patience/clients, health care providers, care coordinators, health plans, and other service systems critical to health.
Preferred Qualifications
- Bachelor’s Degree in Healthcare Management, Public Health, or related field.
- Three (3) years’ experience in managing government grants and contracts at a locate, state, and federal level.
Knowledge and Skills
- Demonstrated strong understanding of how to work effectively with people with chronic disease, mental illness, disability, substance abuse and other health problems.
- Demonstrated and detailed knowledge of the health care system, key service providers and health care delivery system developments as related to the health needs of residents.
- Communicate effectively both verbally and written to a diverse group of residents, staff, and external contacts while maintaining compliance with privacy laws.
