Position Summary: the Community Liaison is responsible for liaising between medical providers, hospitals, congregate housing facilities, social service providers and the community at large to promote public and professional awareness of United Hospice (UH). Provide patient education on the hospice benefit and required documentation when scheduled.
Responsibilities
- Directly serves as the United Hospice liaison between medical providers, hospitals, congregate housing facilities, social service providers, the community at large, and any other type of referring agencies.
- Directly responsible for creating demand for the organization’s services through the effective application of ethical marketing principles and outreach/education methods. In collaboration with the Director of Business Development, the Liaison presents a positive and professional public image of the organization within the community.
- Responsible for the implementing the organization’s strategic growth plan, enhancing UH’s brand/image, effectively managing their assigned territory and partners, and works to achieve departmental and organizational business development goals. Liaison must be able to demonstrate tact, resourcefulness, patience, and dedication in a fast-paced public facing environment. Able to travel to medical, congregate housing facilities and community sites as required daily.
- Develop and communicate strategies that promote awareness of the hospice philosophy to the community. Knowledgeable and able to remain current on hospice eligibility requirements. Proficient in understanding health insurance reimbursement for hospice and comfortable providing patients with detailed overviews.
- Subject matter expert on Medicare Hospice Benefit.
- Educate referral sources on the components of hospice services.
- Educated on patient onboarding requirements and associated forms. Executes necessary patient admission paperwork when necessary.
- Maintain coverage in growth territories. Generate monthly statistical reports and able to translate data into strategies to positively impact growth expansion.
- Identify and open new account opportunities for business development team.
- Reevaluate continually marketing strategies and advise on suggested changes. Execute marketing plan and targeted lists to generate referrals.
- Able to independently develop presentations, execute delivery and work both in person and remotely via computer.
- Draft monthly narrative reports, daily contact notes and follow up.
- Deliver plans of care to physicians for signature and ensures timely return.
Desired Skills and Experience
- Bachelor's Degree in a health area, marketing, business administration or related field or equivalent combination of education and experience. Master’s degree preferred.
- Minimum of two (2) years minimum documented experience in referral development, account management, sales growth, and physician relationships. Experience may be evaluated on case-by-case basis in lieu of a degree. Preferred: Prior marketing and/or sales experience within a hospice/health care setting preferred.
- Proven track record developing patient referrals.
- Must have a valid driver’s license. Comfortable traveling 80 percent of the time and driving in all weather conditions.
- Computer/technology savvy. Able to work in a mobile office environment.
- Strong written and oral communication skills. Able to present hospice benefit, eligibility and organizational pitch with ease and comfort.
- Experience using a CRM database.
- Data analytical skills to customize communication to target audience.
- Skilled presenter. Expert PowerPoint, Access, and Excel user. Experience working in an EMR.
Salary Range for this Position is $50,000 to $60,000
