About the Department

This is a fantastic opportunity for a Licensed Practical Nurse who is dedicated to supporting our mission of delivering patient-centered medical care, demonstrating adaptability in various situations, and collaborating effectively within a multi-disciplinary environment.
Position Duties
1. Interview patients to obtain past and present medical information and record data according to the format and procedures of Falls Community Health, recognize nonmedical needs of patients and their families, and make appropriate referrals to other agencies as necessary. Anticipate patient and provider needs, room patients, and consistently complete vitals, medical history, screenings, clinical events, and medication reconciliation for each patient.
2. Work as a member of a patient care team and provide patient-centered care. Coordinate internal and external referrals. Initiate discussions on self-management goal setting. Participate with quality management work groups and teams to improve patient population health outcomes. Effectively communicate with representatives of diverse and vulnerable populations.
3. Assist attending physicians with required medical services. Perform clinic procedures, including EKGs, ear irrigations, spirometry, peak flows, nebulizer treatments, and phlebotomy. Draw up and administer immunizations. Provide care under the direction of physician or registered nurse.
4. Perform physical assessment measurements and collect laboratory specimens.
5. Provide patients with information and explanation regarding illness, treatment, medication, and follow-up care; field patient questions and problems; and provide physicians access dependent upon judgment of the situation. Triage medical requests and schedule clinic appointments; and answer questions regarding billing, insurance, Medicaid, health care concerns, and relay information to appropriate clinic staff.
6. Attend in-service training, staff meetings, and workshops and utilize outside resources available to maintain professional nursing skills.
7. Perform other such duties and functions as are necessary and incidental to the proper performance of this position.
Minimum Qualifications
Graduation from a licensed practical nurse program with a current and valid licensure as an LPN in the state of South Dakota. Minimum of one (1) year’s clinical or hospital nursing care experience preferred; or any such combination of education, experience, and training as may be acceptable to the hiring authority. Prior experience with electronic medical record systems is desirable.
Other Qualifications
Ability to provide complete nursing services within the LPN scope of practice.
Ability to serve and communicate with a wide variety of clients, including infants, youth, adults, low-income patients, and culturally diverse populations.
Ability to determine and accurately record medical data and service charges electronically while maintaining confidentially.
Ability and willingness to participate in training and informational meetings and functions.
Ability to maintain confidentiality.
Knowledge of community agencies available for patient referrals.
