Description
We are seeking a Nurse Case Manager to join our team at the Hanford Occupational Medical Services Clinic. In this role, you’ll coordinate and deliver case management services for workers with occupational injuries, illnesses, and exposures. You’ll be a key member of the Legacy Health Program team, supporting employees in returning to work safely and navigating their care.
This position is a great fit for a nurse who values clinical independence, structured weekday schedules, and the opportunity to impact patient care in an occupational health setting.
What You’ll Do
- Serve as the clinical point of contact for the Legacy Health Case Management Program.
- Coordinate care plans with patients, providers, and employers to support safe, timely return to work.
- Provide education and counseling to patients regarding medical results, restrictions, and next steps.
- Collaborate with physicians, behavioral health, and other providers to ensure continuity of care.
- Prepare reports, maintain records, and support compliance with program requirements.
- Participate in exposure response and support patient follow-up after workplace incidents.
- Deliver compassionate, patient-centered care consistent with nursing standards and site policies.
Requirements
Qualifications
- Bachelor of Science in Nursing (BSN) – required
- Active Washington State RN license – required
- BLS and ACLS – required (or ability to obtain within 6 months of hire)
- 1-3 years of nursing experience, including case management or occupational health
- Strong communication skills and ability to collaborate across multiple stakeholders
- Proficiency with Microsoft Office and experience using electronic health records (Cority preferred)
- U.S. Citizenship and ability to obtain a DOE security clearance
Preferred
- Certified Occupational Health Nurse (COHN)
- Certified Case Manager (CCM)
Schedule Options
We offer three full-time schedule options to fit your work-life balance:
- 4/10s – Monday through Thursday
- 9/80s – every other Friday off
- 5/8s – standard Monday through Friday

Work Location
Work is primarily onsite at the Hanford Occupational Medical Services Clinic in Richland, WA, with occasional remote work as needed.

TYPICAL PHYSICAL DEMANDS & WORK ENVIRONMENT
Working Conditions
Work will mainly be performed in the general medical offices of the Hanford Occupational Medical Services Clinic. Shifts and locations may rotate between the clinics at 1979 Snyder, Richland, WA and 200 West on the Hanford site, or other locations identified by leadership. Duties will involve frequent interactions with other employees, customers, clients, and vendors. Occasionally, work may be needed outside the typical schedule to accommodate client and/or operational needs. Opportunities for local travel may occur.
Physical Abilities
Able to walk, sit, and stand for extended periods of time; reach with hands and arms; balance; bend; stoop; talk and hear; have sufficient visual ability and manual dexterity to operate a keyboard, computer, calculator, telephone and other such office equipment as necessary; occasionally move and/or lift up to 25 pounds or more with assistance.

Clear and conceptual thinking ability; excellent judgment, troubleshooting, problem solving, analysis, and discretion; ability to maintain composure while managing work-related stressors; ability to handle multiple priorities simultaneously; and ability to meet deadlines reliably. Pay Range: $47-$50/hr
