Position Summary
The Case Manager provides client-centered case management for participants in A Safe Haven’s No Place to Stay (NPTS) program, serving justice-involved individuals on electronic monitoring through a partnership with Cook County Circuit Court's Office of the Chief Judge, and for residents in our Recovery Home programs. This role supports clients in navigating legal obligations, recovery goals, and reentry into the community while addressing barriers to housing stability, employment, and overall self-sufficiency.
Key Responsibilities
1. Client Engagement & Assessment

Conduct intake interviews, verify program eligibility, and perform comprehensive assessments of needs, strengths, and risks.

For NPTS clients, coordinate with court representatives, probation officers, and other justice partners to ensure compliance with legal and electronic monitoring requirements.

Develop individualized service plans addressing housing, recovery, legal compliance, employment, education, and health needs.

Establish a supportive and professional relationship with each client, using a trauma-informed and culturally responsive approach.
2. Ongoing Case Management

Meet regularly with clients to review progress toward service plan goals and compliance with program and legal requirements.

Connect clients to appropriate A Safe Haven services and external resources, including recovery supports, vocational training, mental health services, medical care, and legal assistance.

For Recovery Home residents, coordinate care that supports sobriety, relapse prevention, and sustainable reintegration into the community.
3. Documentation & Compliance

Maintain accurate, timely, and confidential case records in compliance with program, court, and funder requirements.

Track and report client participation, legal compliance, and progress toward goals in designated databases.

Immediately report violations of program or court requirements to the Program Manager and, where required, justice system partners.
4. Program Coordination

Participate in multidisciplinary case conferences with internal staff and external justice partners to coordinate services and address client challenges.

Support client transitions, including moving from NPTS to independent housing or from Recovery Home to less intensive services.

Assist with bed management, admissions, and discharges in coordination with the Office of the Chief Judge and referral partners.
5. Advocacy & Support

Advocate for clients within the justice system and community agencies to promote access to services and fair treatment.

Provide crisis intervention and problem-solving support, including addressing barriers to compliance and safety concerns.

Promote a structured, recovery-focused environment built on accountability, dignity, and empowerment.
Qualifications
Required:
-
High school diploma or GED.
-
Minimum 2 years of experience in case management, counseling, or related work with justice-involved individuals, people experiencing homelessness, or those in recovery.

Knowledge of court processes, probation/electronic monitoring requirements, and reentry challenges.

Strong communication, problem-solving, and organizational skills.

Proficiency with Microsoft Office and case management systems.
Preferred:
-
CADC, CRSS, NCRS or equivalent credential.

Experience in a Recovery Home or residential reentry program.

Bilingual (English/Spanish) skills.
Work Environment & Physical Demands

Fast-paced residential and justice-coordinated program setting.

Requires ability to sit, stand, and walk for moderate periods.

Requires occasional evenings, weekends, or holidays to meet program and court-related needs.
