This position provides basic social services to individuals and/or families participating in the Healthy Start Program in Lake County. The responsibilities include case management for Healthy Start clients and education, and ensuring clients receive appropriate referrals and services in compliance with Healthy Start Guidelines.   Funding for this position is provided by the Healthy Start contract to improve pregnancy outcomes.

ESSENTIAL FUNCTIONS: 

  • Answers telephones, schedules appointments, and takes and relays messages in a professional and timely manner.
  • Carry work-provided cell phone and be available to receive any office phone calls and record messages, check main office phone messages.
  • Assist in developing and maintaining electronic tracking tools necessary to the Healthy Start program.
  • Maintain inventory trackers and assist with inventory counts.
  • Physically move and organize inventory in the storage unit and office, and rectify invoices with inventory counts.
  • Maintains organization of office supplies and educational materials. With approval, replenishes as necessary.
  • Helps drop off supplies to clients’ homes if needed, diapers, wipes, Pack N Plays, etc.
  • Under the supervision of the Healthy Start Supervisor, provide direct support to clients by providing resources and referrals. 
  • Under the supervision of the Healthy Start Supervisor, complete initial assessments face-to-face with clients in their homes or alternate locations as required by the Healthy Start Standards and Guidelines and core performance measures.
  • Prepare and facilitate group connections as required. Encourage clients to attend.
  • Attend unit meetings and take meeting minutes.
  • Assist clients in accessing community services and provide information and education to clients about smoking cessation, childbirth education, breastfeeding support, interconception education and reproductive life planning, parenting, and infant growth and development.
  • Processes incoming referrals and intakes through fax and the Well Family System (WFS) within the required time frames.
  • Must maintain positive working relationships with other community agencies and medical providers.
  • Maintains/ensures the Healthy Start database is updated promptly.
  • Construct new client folders and complete an initial client packet, which includes assessments, family support plans, release of information, etc, with new clients at the time of Initial Assessment.
  • Upload and document initial client packet in WFS and assign cases to Care Coordinators. 
  • Assist in conducting outreach activities to providers and other community organizations to promote the Healthy Start program.
  • May conduct client satisfaction surveys as related to the Healthy Start guidelines and protocols.
  • Attend meetings, classes/conferences, and in-service programs as requested or required to update knowledge and skills related to the Healthy Start Program and other related activities.
  • Assist with the completion of monthly and quarterly reports.
  • Assist in the completion of required quality assurance activities as designated by the Supervisor.
  • Assist in Care Coordination services as needed to help manage caseloads. 
  • Other duties as assigned.

QUALIFICATIONS: 

  • Education/Experience

An associate’s degree in Human Services, Social Work, or a Health-Related Field such as Health Education, Child Maternal Health, or Public Health is preferred. High School Diploma required. Two (2) years of related experience preferred.  Experience conducting home visits with a similar population is preferred. Bilingual (Spanish) preferred. 

  • Licensure/Certifications

Valid Florida driver’s license and insurable by the agency’s insurance carrier. 

  • Other Requirements

Must successfully clear Level II Background Screen

Must possess an acceptable driving record and have reliable transportation.

KNOWLEDGE, SKILLS & ABILITIES:  

  • Excellent knowledge of community resources
  • Excellent interpersonal and communication skills (verbal and written)
  • Excellent computer skills in Microsoft Office Suite (Word, Excel, PowerPoint)
  • Effective public speaking skills
  • Ability to adapt to a virtual working climate as needed
  • Ability to travel up to 75%
  • Ability to safely operate a motor vehicle
  • Ability to lift up to 20 lbs.
  • Ability to work evenings and weekends as needed
