MAJOR FUNCTIONS:

The Outpatient Counselor is responsible for providing counseling and case management services related to substance abuse, legal, mental health, medical and vocational/educational issues. The Outpatient Counselor documents all provided services in a manner that is compliant with regulatory and third-party payor requirements. She/he works to coordinate and provide treatment services based on the individualized treatment plan including discharge planning. The Outpatient Counselor advocates for and promotes a recovery-based lifestyle. The Outpatient Counselor works closely with the rest of the staff as an integral part of the clinical team.   

   

SPECIFIC DUTIES & RESPONSIBILITIES:

  1. Administer comprehensive psychosocial assessment tools at intake and at various points in accordance with Odyssey House policy and procedures. 
  2. Provide individual counseling, group counselling and case management services onsite and in the home or community.
  3. In collaboration with each client, formulates a comprehensive individualized treatment plan, monitors achievement of objectives, reviews and updates the treatment plan in accordance with clinical needs of the client. 
  4. Maintain up-to-date client clinical records including at minimum: treatment planning, group counseling notes, progress notes, and the monitoring of social determinants of health in accordance with regulatory guidelines and program standards.  
  1. Provide crisis intervention services.
  2. Responsible for ongoing client discharge planning beginning at admission. 
  3. Provides ongoing assessment of each assigned client, monitoring for any changes in clinical needs.
  4. Assures timely an effective care coordination with collateral services providers, family members and criminal/family justice agencies. 
  1. Delivers clinical services in accordance with evidence-based curricula and training.
  2. Acts as a liaison with all referral agencies providing regular verbal and written progress reports.
  3. Establish community and agency affiliations which address the client’s need and strengthen the delivery of services.
  4. Provide education and direct services to family members, as appropriate, regarding client’s substance use/misuse and related behavioral issues.
  5. Conduct collection of samples for toxicology randomly and as needed.  
  6. Attend regularly scheduled staff meetings and case conferences.
  7. Attend all required in-service training seminars.
  1. Participate in quality improvement activities. 
  1. Other relevant duties as required. 

REQUIREMENTS:   EDUCATION/TRAINING AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES

  1. CASAC or CASAC-T preferred, or the ability to obtain within 6 months of hire. 
  2. Ability to deliver culturally competent services to a diverse client population in an urban setting.
  1. Proficiency with computer operation (Microsoft Word, Excel, Electronic Health Records, and Outlook programs). 
  1. Must be able to work a flexible schedule.
  2. Bilingual Spanish preferred
