Job Summary

Position will lead an innovative Community Health Worker (CHW) Hub designed to improve health outcomes for high-risk patients impacted by social drivers of health (SDoH). This role will leverage contact center operating principles, technologies and data-driven workflows to proactively connect patients to essential community resources and support services.

The ideal candidate brings a strong background in business operations, contact center operations, process improvement and/or population health with a passion for harnessing technology to scale operations. As the leader of the CHW Hub, they will design and implement operational strategies that optimize performance, enhance access, and ensure culturally and linguistically appropriate engagement with vulnerable populations.
The position will play a critical role in integrating social care into healthcare delivery, managing a dynamic team of CHWs, and collaborating cross-functionally to drive equity, innovation, and measurable impact across our system.

The leader will report to the AVP of Ambulatory Contact Center and serve as an operational partner of the Inpatient Navigation, ER Navigation, Community Resource Center, Neighborhood Health Center and Health Center for Schools programs. The Hub is designed to support efficient referrals and, when applicable, appointment setting to the listed community programs. The manager will act as a collaborative partner to each of the management teams of the respective programs.

Job Description

Minimum Qualifications

Education:  Bachelor's degree in Healthcare Administration, Public Health, Social Work, Nursing, Business or related field required; masters preferred.

Licenses/Certifications:  (None)

Experience / Knowledge / Skills:

  • Minimum of five (5) years of experience with proven track record in program development, experience facilitating complex, multi-faceted grant projects with multiple collaborators and networking with industry community organizations and government agencies.
  • Three (3) years of direct management experience in not-for-profit community work, preferably in healthcare.
  • Ability to research, analyze, problem solve, and evaluate.
  • Ability to lead and implement change through the development team.
  • Ability to work collaboratively with health care professionals and community partners at all levels to achieve established goals and improve outcomes.
  • Strong computer proficiency including working knowledge of Microsoft Products (Office, Word, Excel, Outlook and PowerPoint).

Principal Accountabilities

  • Plans, organizes, and directs achievement of objectives through innovative ideas and a disciplined management approach.
  • Compiles and presents analytical information to illustrate how the investment in development initiatives contributes to overall CBC objectives.
  • Responsible for overall development and implementation of strategic plans in order to establish Memorial Hermann as a community collaborator and innovator for its various community programs.
  • Collaborates to promote Memorial Hermann’s effective work in the community and maintains an in-depth knowledge and rationale behind the strategy, plans, programs, and critical issues.
  • Partners and collaborates at the community level to represent Memorial Hermann and participate on the behalf of Memorial Hermann Community Benefit Programs.
  • Responsible for direct performance management of Community Outreach, Education and assigned CBC Program staff as relates to quality of: outcomes; volumes; and community penetration.
  • Ensures positive employee relations and the development to a competent, positive and motivated work force.
  • Designs and implements measures to evaluate existing as well as proposed initiatives, including clinical, behavioral, and social factors.
  • Responsible for effective presentation of Community Benefit Corporation vision and activities through website presentation, award submissions, and internal marketing, and performs as liaison with Corporate Marketing.
  • Partners with the administrative and staff levels within the school districts, hospitals, and community to collaborate for successful outcomes.
  • Serves as a liaison with marketing.
  • Responsible for effective communication to senior leaders and collaborative partners through personal contact, presentations and reporting.
  • Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
  • Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
  • Demonstrates commitment to caring for every member of our community by creating compassionate and personalized experiences. Models Memorial Hermann’s service standards by providing safe, caring, personalized and efficient experiences to patients and colleagues.
