Diabetes and Nutrition EducationOverview of Position:
This position is responsible for implementing education programs for Genesis HealthCare System employees and the community, specifically related to diabetes education. Coordinates the implementation of programs utilizing sound adult learning principles. Acts as a liaison to all departments and is available to travel to various units to assist with competencies, train preceptors and assess education needs for the unit/department.
ESSENTIAL DUTIES
1. Collaborates with peers to research, plan, and implement education programs that reaches the Genesis six county region
2. Assesses learning needs of inpatient and outpatient clients within the organization
3. Provides inpatient education and coordinates a successful transition from inpatient to outpatient diabetes services
4. Collaborates to research current practices. Plans and implements educational offerings throughout the Genesis six county region.
5. Based on quality improvement studies, risk management reports and low volume high-risk procedures, identifies annual competencies to maintain quality care of patients.
6. Assesses socioeconomic indicators for successful behavior change, designs education, and performs appropriate referrals and follow-up as needed.
7. Facilitates both individual and group sessions and maintains positive outlook regardless of content.
8. Develops and maintains departmental policies and procedures.
9. Maintains contact with regulatory or credentialing body to maintain current standards (i.e. AHA, ADA, AADE, AACE/ACE) and completes any necessary requirements
10. Maintain knowledge and actively assists peers with ADA certification requirements and application processes
11. Engaged and actively participates in PHW division Shared Governance Council
12. Assists with new employee classes and competencies.
13. Travels to various units to assess learning needs of new employees, preceptors or established employees. Assists with implementing best practice on the units.
14. Completes documentation within established guidelines to ensure timely processing for billing or reporting.
QUALIFICATIONS
1. BSN or Bachelor’s degree in a health-related field and current Ohio RN License
2. BLS required or obtained upon hire
3. Certified Diabetes Education (CDE) acquired within 12 months of meeting all prerequisites for certification; Maintain CDE requirements for recertification
4. Certified Pump and CGM acquired within 24 months of hire
5. Excellent interpersonal, decision-making, conflict resolution and investigative skills
6. Ability to apply adult learning principles to the classroom and other learning experiences
7. Demonstrated excellence with listening, verbal and written communication skills
8. Ability to work independently and under pressure in a complex and changing working environment
9. Excellent computer skills and knowledge of computer software, including programs such as Word, Excel, PowerPoint, etc.
10. Knowledge of audio-visual equipment (laptop, VPU, Overhead) and able to select equipment best suited for class

WORKING CONDITIONS/PHYSICAL REQUIREMENTS

1. Works in an office and or classroom environment.
2. Ability to read, comprehend and calculate information
3. Ability to stand, walk sit, stoop and bend for extended periods of time.
4. Ability to operate a computer for 2-3 hours at a time.
5. Ability to drive to meetings and travel within the Genesis 6 county region for speaking engagements/education and assist with education of employees.
6. Regularly lift or move up to 10 pounds and frequently lift or move up to 25 pounds.
7. Vision abilities required include up close vision, peripheral vision, depth perception and the ability to adjust focus.
8. May be required to periodically rotate shifts and regular days off. All system employees must be willing to work all shifts, extra hours, holidays and emergency shifts as required.
