SUPERVISORY RESPONSIBILITIES 

Provide direct supervision and training to assigned staff, volunteers and interns.

SUMMARY 

Under the direct supervision of the assigned supervisor, the  Coordinator is responsible for the direct coordination of CalAIM Navigators and coordinates the staff providing CalAIM services embedded within other programs. Coordinator is responsible for productivity management, budget compliance and overall compliance with CalAIM standards.

DUTIES AND RESPONSIBILITIES/ESSENTIAL FUNCTIONS

Responsible for supervising assigned staff, interns and volunteers; recruitment, performance management and other personnel processes.

Responsible for the overall implementation and ongoing operations of CalAIM services including Enhanced Case Management (ECM) and Community Supports (CS) including Housing Tenancy Services and Housing Navigation Services.

Coordinates with Health and Dental Services staff to ensure referrals are received, contacted, and that all case notes are provided to ensure billing compliance.

Responsible for ensuring a housing assessment has been completed for each client, which includes short and long-term measurable goals.

Responsible for assisting clients in searching for and securing housing, which includes providing transportation either by bus passes or directly transporting clients in program vehicles.

Responsible for assisting clients with communication and advocacy with landlords to secure and/or sustain housing.

Responsible for assisting with benefit advocacy and linkages to ensure assistance is obtained for necessary mainstream benefits

Coordinates case reviews of all CalAIM clients, at least once per month, and more frequently as needed.

Prepares and submits activity reports on outcomes achieved as directed. Reports include but are not limited to weekly, monthly, quarterly, semi-annual and annual reports.

Assists in the collection and entry of data, as required, in project’s Management Information System. 

Develops and maintains effective working relationships with community service providers and/or collaborative partners.

Engages in relevant community meetings; participate in councils, roundtables and committees, as needed.

Provides community outreach, training and public awareness regarding program services.

Conducts and compiles satisfaction surveys.

Coordinates and assists in direct advocacy and client services in crisis intervention, including trauma-informed response.

Adheres to the confidentiality policy regarding participants.

Travels for agency business using personal/agency vehicle.

Maintains regular attendance and punctuality.

Works evenings and weekends as required.

Performs other duties as assigned.

QUALIFICATIONS

EDUCATION:

  • High school diploma or GED.
  • Bachelor’s Degree in Social Work, Psychology, Sociology, Counseling, Human/Child & Family Development or related field required. Additional relevant work experience may substitute required education on a year-by-year basis.

EXPERIENCE:

  • Minimum of two years’ experience in community outreach, public speaking, and volunteer coordination required. 
  • Minimum of two years’ experience working with homeless individuals and homeless families is highly desirable.
  • Minimum of two years related experience demonstrating the ability to effectively supervise staff, and carry-out the duties/essential functions of this job.

OTHER QUALIFICATIONS:

Must be at least 18 years of age.

Proficient in Microsoft Office Suite and related business software.

Effective presentation, written and verbal communication skills.

Ability to write reports and resolve conflict.

Ability to support, network and motivate volunteers.

Ability to organize and maintain detailed records; complete necessary reports and meet deadlines. 

Bilingual English/Spanish preferred based on the needs of the clients and the program.

Ability to inspire and motivate participants to help overcome barriers. 

Ability to exercise discretion and independent judgment.

Willing to work non-traditional hours and days to meet the needs of this position.

CPR and First Aid certification preferred; required within six months of hire.

Pass required background investigation.

Maintain valid California driver's license and the minimum auto insurance as required by state law, or reliable transportation. For those driving agency vehicles, must maintain insurability under agency policy.
