Position Summary:

Forward facing, the RCC is in relationship with residents, caregivers, and volunteers leading by example by providing exceptional care to residents. This includes all stages of the resident progression: pre-admission screening, the admission physical, care, education and training of staff and volunteers, and working on nursing issues with hospice. Work with residents/family members, certified home health agencies, licensed hospice, social workers and discharge planners. Maintain compliance with all regulatory standards.

Essential Job Duties and Responsibilities: (Additional duties may be assigned)

· Establish relationships with residents and family members upon admission and throughout the admission process providing the highest degree of care and emotional support ensuring the mission, vision, and values of Francis House are followed in order for all to feel surrounded by unconditional love of God in a peaceful, serene environment in a homelike environment.

· Provide quality care and services to residents and their families ensuring that resident needs are addressed and they are treated with respect and dignity. The Resident Care Coordinator will act in a caregiver role as needed and provide hands-on care, physical and emotional support while maintaining a safe and comfortable home-like environment. 

· Demonstrate a high standard of nursing practice related to end-of-life care and pain and symptom management through competency evaluations. Address resident needs on admission through rounding, caregiver and/or family and resident huddles, and safety briefings, and monitor daily for changes in condition that are reflected in the care plan. Maintain accurate and detailed healthcare records so that any problem, plan of care, action, or goal are accurately and clearly stated, and changes are reflected as they occur and update medical staff and family members as needed. Maintain accuracy in and storage of charting, incident reports and audits. 

· Provide information to potential residents and family members regarding Francis House, including the referral pool and possible admission, offering beds when available. Update referral pool as needed including entering data into database. 

· Meet with administrative and staff members to seek input, review concerns, share professional support and education, and optimize communication to enhance the quality of resident care and professional growth. 

· Ensure safe levels for all shifts. Consistently monitor schedule and fill open shifts appropriately. 

· Work with the director, volunteer manager, and house coordinator to create meaningful opportunities for volunteers 

· Initiate resident log on admission and update as needed. Coordinates admissions and interdisciplinary rounds efficiently.

· Ensure durable medical supplies and medications are available. Create medication profile and pre-pour medications according to Francis House standards upon admission and as needed including collaboration with Hospice to ensure medication profile appropriateness for pre-poured medications under the direction of the RCC and administered by unlicensed care staff.

· The Resident Care Coordinator will participate in regular on-call rotations as part of the role and must maintain readiness and availability during designated on-call shifts.

· Must be available to work at least two (3) weekends annually. 

Resident Care/Service Accountabilities:

Supervisory Responsibilities:

This position supervises the Care Team Scheduler/Preceptor, Caregivers, and On-Call Administrators. 

Education and Experience: 

· Graduate from an accredited school of nursing.

· Master’s degree in nursing preferred.

· Registered nurse in New York State. 

· Five to eight years’ experience in professional nursing in a hospital, hospice, homecare, or nursing home environment.

Special Requirements: 

· Valid NYS drivers’ license required.

· Occasional local travel during regular work hours.

· Shift work.

Knowledge, Skills, and Abilities:

· Basic computer skills including the ability to send/receive e-mail messages and perform basic word processing and/or data entry.

· Ability to work with and function as a team member at all levels within the organization to include fostering growth in caregivers through regular real-time education, mission feedback and support. 

· Fosters and role models relationship with Francis House residents and families, Volunteers, caregivers, and administrative staff.

· Ability to respect the dignity of all who serve and are served.

· Ability to represent Francis House effectively in a variety of settings and with diverse communities.

· Ability to educate residents and/or families as to the nature of disease and to provide instruction on proper care and treatment. 

· Assist families to find meaning and closure, emotional awareness, and decision making in actively advocating care decisions for their loved ones. 

· Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.

· Holistic interventions for residents and families. 

· Knowledge of patient care charts and patient histories.

· Ability to observe, assess, and record symptoms, reactions, and progress.

· Maintains a watchful eye on standards of resident care and safety outcomes. 

· Leads quality initiatives using metrics like falls, pain, safety and symptom management to guide improvements. 

· Maintains quality dashboards, with benchmarks, for tracking outcomes.

· Monitors budget, via overtime, attrition, time and attendance and expenditures. 

· Ability to develop, implement, and modify multidisciplinary healthcare plans that reflect how we can make Francis House a special place at the end of their life to make ones passing memorable to residents and family. 

· Ability to make administrative/procedural decisions and judgments.

· Knowledge of clinical and social services documentation procedures and standards.

· Sustain morale through change, loss, or system strain. 

Equipment Used:

· Computer, printer, photocopier, telephone, fax machine, calculator, scanner and a variety of medical equipment and instruments including, but not limited to hoyer lift, stretcher bed and wheelchair.

· Computer Software: Microsoft word, Outlook, PowerPoint and Excel, Paylocity Payroll Software. 

· Safety Equipment: Work environment involves exposure to potentially infectious diseases and unpleasant tasks that require the use of latex or vinyl protective gloves, gown and mask.

Physical and Mental Requirements:

· Ability to sit, stand, walk, talk, hear, use hands to finger, grasp, handle or feel, and perform repetitive motions of the hands and/or wrists; push, pull, reach, stoop, bend, kneel and climb.

· Exerts up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently or constantly to move objects.

· Close mental and visual attention required to perform machine operations and set up, read, and document resident information.

Environmental Conditions:

Work is performed in a typical medical environment. Duties include handling garbage or soiled linen. There is substantial exposure to blood, body fluids and/or pathogens, infectious diseases, unpleasant odors and hazardous materials that require following extensive safety precautions and include the use of protective equipment.
