About the Role
We are seeking dedicated and organised Care Brokers to join our team. The successful candidate will be responsible for sourcing and facilitating the setup of care support services for users on a managed service. You will contribute to a local knowledge base of care services and ensure that all information is maintained and recorded appropriately within IT systems.
About You
To apply for the Care Broker post, you must:
- Respond to requests from commissioners to find suitable community care service options, maintain regular contact with care providers and keep knowledge of service capacity up to date.
- Place orders for care services with providers and supply them with all necessary information.
- Ensure all information related to the placement of care is accurately recorded.
- Monitor supply and demand issues within a specified locality and liaise with commissioners and providers to ensure priority needs are met.
- Report any concerns about the quality or availability of service provision to the Quality and Contracts Team.
- Maintain accurate records and adhere to relevant policies and procedures.
- Work effectively as a member of the Brokerage Support Team and provide cover for absent colleagues and attend team meetings and supervision sessions constructively.
