PURPOSE OF THIS POSITION
The primary purpose of this job is to oversee and evaluate Bridge Quality Assurance activities, compliance, and education in collaboration with BVHS and department leadership for educational opportunities in accordance with all current applicable federal, state and local standards, guidelines and regulations and the requirements of all home care regulatory bodies.
JOB DUTIES/RESPONSIBILITIES
Duty 1: Manages the activities of the Bridge Quality and Intake Departments. Has primary responsibility for identifying and using proven performance improvement techniques to address opportunities for improvement (OFI) in both clinical quality and compliance effectiveness across Bridge service lines.
Duty 2: Collaborates with Bridge Leadership to oversee the clinicians in performance of their daily duties and assists clinicians with assessment/intervention/evaluation skills.
Duty 3: Ensures that the requirements of applicable governmental, licensing and accrediting agencies are met for Bridge services.
Duty 4: Plans and participates in the orientation of new associates and in the ongoing education of current associates.
Duty 5: Oversees the tracking, aggregating and submission of data pertaining to current process, quality measures and other benchmarking data to state, federal, and contracted agencies.
Duty 6: Collaborates with the Bridge Leadership and outside sources as needed in the development and administration of educational programs for Bridge associates including quality, clinical, documentation and other subjects as necessary.
Duty 7: In collaboration with Operations Director, develops the quality budget and educational budget for Bridge.
Duty 8: Documentation is consistent with or exceeds Bridge guidelines.
Duty 9: Displays Service Excellence as evidenced by practicing the mission, vision, and values of the organization to promote patient satisfaction. Complies with organizational policies, procedures, and practices to ensure quality job performance and Medicare/Medicaid compliance.
Duty 10: Functions as a member of the Bridge leadership team providing overall direction for Bridge service lines, reporting to the Bridge Director of Operations.
Duty 11: Participates in state and regional professional and trade organizations as appropriate.
Duty 12: Participates in Administrator on-call rotation as scheduled
REQUIRED QUALIFICATIONS
Possesses a clinical license of RN, PT, or OT.
1-2 years of general field experience.
Commitment to and knowledge of the philosophy, mission and goals of the agency and standards and governing regulations.
Prior experience in compliance, quality, and performance improvement activities.
Proven management skills.
Reliability, flexibility and excellent organizational skills.
A valid driver's license is required (if you do not have a valid Ohio driver’s license you must obtain one within 30 days of your residency in the state). You must also meet BVHS's company fleet policy and insurance requirements, and any other requirements that may be required to operate a vehicle.
Positive service-oriented interpersonal and communication skills.
Individual must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served on his/her assigned unit/department. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient status. Must be able to interpret the appropriate information needed to identify each patient’s requirements relative to their age-specific needs and to provide the care needed as described in the area’s policies and procedures.
PREFERRED QUALIFICATIONS
Minimum of three (3) years of Home Care experience.
PHYSICAL DEMANDS
This position requires a full range of body motion with intermittent walking, lifting, bending, climbing, squatting, kneeling, twisting, sitting, and standing. The associate will be required to walk up to two hours a day, sit for four hours a day, and stand for two hours a day. The individual must have good eye/hand coordination and fine finger dexterity. The individual must have excellent verbal communication skills to perform daily tasks. The associate must have corrected vision and hearing in the normal range.
