PURPOSE OF THIS POSITION

The Bridge Supervisor - Home Health Therapy is responsible for supervising the home health care provided to Bridge Clients by the therapy team. The Home Health Team Supervisor oversees the planning, coordination, and delivery of direct care provided by the Home Health Therapy Team through care plans, OASIS, scheduling, document review, and direct supervision of the therapy team members.

JOB DUTIES/RESPONSIBILITIES

Duty 1: Maintains own caseload of patients, including, but not limited to: gathering of information, performance of patient assessments at required timepoints, development of care plans and goals, and completion of patient treatments, interventions, and education, as applicable per discipline.

Duty 2: Demonstrates the knowledge and skill to mentor the Home Health Therapy Team in providing consistent quality patient care to those being referred to or receiving Home Health Care services.

Duty 3: Coordinates schedules of therapy team members in relation to the Plan of Treatment. Assists with assigning primary patients and/or visits per Bridge Home Health and Hospice Guidelines.

Duty 4: Coordinates with Quality Manager, Community Nurse Liaisons, and information management specialist to supervise work of contract therapy groups to ensure compliance with documentation, care plans and goals, associate files, and other regulatory requirements.

Duty 5: Supervises the clinicians in performance of their daily duties and assists clinicians with assessment/intervention/evaluation skills; reports concerns to Home Health management.

Duty 6: In collaboration with Home Health management and clinical education, plans and participates in the orientation of new associates and in the on-going education of current associates, and facilitates interdisciplinary team meetings and communication/collaboration within the Home Health Care teams.

Duty 7: Ensures personal and therapy staff’s documentation is consistent with or exceeds Bridge Home Health and Hospice guidelines.

Duty 8: Coordinates with Home Health management for oversight of Home Health department, including staff performance evaluations for therapists.

Duty 9: Displays Service Excellence as evidenced by practicing the mission, vision, and values of the organization to promote patient satisfaction.

Duty 10: Complies with organizational policies, procedures, and practices to ensure quality job performance, Medicare/Medicaid compliance to include OASIS, and accrediting body regulations are followed.

Duty 11: Completes other duties as assigned.

REQUIRED QUALIFICATIONS

  • Current state licensure as a Physical Therapist (PT) or Occupational Therapist (OT)

  • Minimum of 1-2 years of general field experience

  • Commitment to and knowledge of the philosophy, mission and goals of the agency, and standards and regulations governing it.

  • A valid driver's license is required (if you do not have a valid Ohio driver’s license you must obtain one within 30 days of your residency in the state).  You must also meet BVHS's company fleet policy and insurance company requirements, and any other requirements that may be required to operate a vehicle.

  • Positive service-oriented interpersonal and communication skills required.

  • Individual must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served on his/her assigned unit/department. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient status.  Must be able to interpret the appropriate information needed to identify each patient’s requirements relative to their age-specific needs and to provide the care needed as described in the area’s policies and procedures

PHYSICAL DEMANDS

This position requires a full range of body motions with intermittent walking, lifting, bending, squatting, kneeling, twisting, and standing. The associate will be required to walk for up to one hour a day, sit for four to six hours a day and stand for one hour a day. The individual must be able to lift twenty to fifty pounds and reach work above the shoulders. The individual must have good eye-hand coordination and fine finger dexterity for simple grasping tasks. The individual must have excellent verbal communication skills to perform daily tasks. The associate must have corrected vision and hearing in the normal range. The individual must be able to operate a motor vehicle for business travel and community involvement.
